0557753317 2017-12-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 had reached total depth in their intermediate section at 6923 feet and 9.625 inch diameter 40 pound per foot casing was set at 6913.95 feet from KB which is 26 feet. The DV tool was set at 3677 feet. O-Tex Pumping was onsite preparing to rig up and complete a 2-stage cement job. I met with Ascent Consultant Cecil Lovell and Cement Specialist Mark Henderson and reviewed upcoming job.
0668862867 2017-09-22 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled the cement out of the 13.375 inch diameter casing, performed a cement bond log and submitted their plan for remediation. Falcon Drilling Rig #26 ran 1 inch diameter tubing to 290 feet. O-Tex Pumping LLC was onsite gaining circulation with 100 barrels of fresh water and pumped a total of 100 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack and 800 sacks of Class A 15.6 pound per gallon 1.18 cubic foot per sack cement at 1.5 barrels per minute. Cement weight was verified by pressurized mud scales. Cement was tagged at 75 feet from surface with Falcon Drilling Rig# 26 pulling 1 inch diameter tubing from the wellbore and operations shut down to wait on cement to set and more cement to arrive.
0794066403 2017-09-20 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1315 feet. Falcon Drilling Rig #26 Wyoming casing crew ran 1309.05 feet of 13.375 inch diameter 61 pounds per foot J-55 casing 5.1 feet above KB. Falcon Drilling Rig #23 washed down the last 9 joints of casing pumping a total of 250 barrels of fresh water. Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 64 degrees. O-Tex was onsite and pumped 5 barrels of fresh water and performed a 1000 pound per square inch pressure test. O-Tex pumped 35 barrels of gel and a planned total of 296 barrels and losing circulation at 160 barrels in to cement. Cement locked up at 254 barrels of the 296 barrels at a pressure of 800 pounds per square inch. Multiple unsuccessful attempts were made to get cement moving again. A meeting was held discussing calculations, estimated top of cement and a plan forward with Ascent Consultants Michael Rollins and Ascent Cement Guru Mark Henderson. Plan forward included: run 1 inch diameter tubing tagging cement in the annulus, gain circulation with water, top out cement to surface, drill out cement, perform cement bond log and perform casing integrity test. 1017572182 2019-04-25 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present with each valve identified with casing information. The 13.375-inch diameter casing had 10 pounds per square inch and the 9.625-inch diameter casing had 150 pounds per square inch. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1382183910 2018-07-06 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of eight 400 barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1506172030 2017-09-23 Casing String Witnessed No Violations On the day of my inspection I witnessed Falcon Drilling Rig #26 tag the cement at 106 feet and run 106 feet of 1 inch diameter tubing. O-Tex Pumping LLC was onsite immediately gaining circulation and pumped a total of 300 sacks of Class A 15.6 pound per gallon 1.18 cubic foot per sack cement at 2 barrels per minute. Cement weight was verified by pressurized mud scales and 16 barrels of cement was circulated to surface weighing 15.3 pounds per gallon. Falcon Drilling Rig# 26 pulling 1 inch diameter tubing from the wellbore.
