0341103679 2017-12-19 Casing String Witnessed No Violations On the day of my inspection I witnessed that Patterson UTI Drilling Rig #577 had drilled a 17.5 inch diameter hole to the depth of 1301 feet. Wyoming Casing Crew ran 1303.47 feet of 13.375 inch 61 pound per foot J-55 casing set at 3.5 feet above KB. KB height is 26 feet above ground level. C&J Energy Services was on site and loaded their lines with 5 barrels of fresh water and performed a high pressure test at 3000 pounds per square inch. C&J Energy Services gained circulation with 35 barrels of gel, 10 barrels of fresh water and cemented casing with 1315 sacks of Class A cement 15.6 pounds per gallon, 1.2 cubic foot per sack cement with 1.5% Calcium Chloride, 3% Salt, .25 pound per sack Cello-Flake and .2% X-

Precision Rathole was onsite and preparing to run and set the 16 inch diameter pipe.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of eight 400 barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, one compressor, one vent scrubber, and one combustor. Identification was located on the well. Upon entering the access road, a slip was noticed developing on the left side of the access road at the point of entry and an additional slip had developed on the stack pile. I made ODNR Engineer Mark Atkinson aware of my findings.
