1196441905 2016-02-25 Plug / Plug Back No Violations On the day of my inspection following up on the initial inspection from 2/22/16, Columbus verified that coordinates match and the records state the well was plugged in January 1931. The well was formerly known as the Speer #3 and it never was a Chesapeake Appalachia LLC well. Upon return to the site I witnessed the hole was 18 inches by 12 inches at the surface and was approximately 14 feet deep with no visible casing present. The land owner stated that they believed that it was a water well and at one time had a concrete slab over top of it. Further inspection revealed no slab near the surface and utilizing an ODNR Honeywell PhD6 meter no gas detected.
1750270170 2016-02-22 Production Wells No Violations On the day of my inspection utilizing a handheld GPS unit, coordinates from RBDMS and thoroughly inspecting the field doing a 300 foot sweep of the radius, I located a hole at (+/-8) 40.016833 -81.656332. I referred the coordinates to Columbus for verification.
