0010089090 2014-09-12 Casing String Witnessed No Violations This well was spud on 9/10/14. Rocky Mountain Drilling drilled 120 of 26 hole and set 120 of 20 casing. Universal cemented it with 250 sacks of Class A cement. See the casing ticket for the details.
0277883656 2015-01-08 Drill / Deepen / Reopen No Violations On the day of my inspection I found that the rig crew had TD the well at a depth of 16997 feet. The were in the process of the 5.5 inch casing.
0720481871 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters. 3 Compressors 2 NGL tanks There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
