0704603730 2019-01-29 Completion Testing No Violations On the day of my inspection I witnessed CWS rig 7009 onsite. This well has had frac plugs drilled out and approximately 8218 feet of 2.875 inch diameter tubing will be ran. Surface annular pressure is 0 and intermediate annular pressure was 300.
0803843628 2015-05-07 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor. I was informed by Precision Rathole that they had drilled a 36 inch diameter hole to a depth of 100 feet. Precision set 100 feet of 30 inch diameter, 118lbs/ft conductor on bottom. I field titrated mix/displacement water with a Quantab and found the level to be 32ppm. Allied Oil & Gas Services pumped 600sk of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl2, and 0.25lbs/sk of flake. Allied displaced cement with 76bbl of water. 20bbl of cement returned to surface and stayed at surface while I was on site.
0913964940 2018-10-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 795 had reached total depth of 18440 feet earlier this morning. I identified myself to company man Larry Hart. He stated that they were circulating on the well and should be cemnting the production casing on Wednesday.
0982858031 2018-03-15 Drill / Deepen / Reopen No Violations On the day of my inspection I found this pad to contain 1 properly labeled producing well and four conductors/cellars that are not labeled. Production equipment includes one GPU, a vapor recovery tower, combustor, compressor, and four 500 barrel steel tanks. Steel tanks are in metal containment and there is a berm around pad. There is no drilling equipment onsite. Outslopes of pad are stable and vegetated. Pad is fenced and locked. Per ORC 1509.072A this permit is not eligible for a preliminary restoration until it is drilled to total depth.
1213738779 2018-12-20 Completion Testing No Violations On the day of my inspection I was accompanied by Geologist Tammy Richards. A halliburton frac crew is on location. Equipment is down for maintenance and no frac is ongoing. Surface annular pressure is 100 psi and intermediate annular is 300 psi.

They just finished perforating stage 2/67. Plug was set at 18229, with bottom perforation at 18220 and top perforation at 18099. Intermediate annular pressure was 0.
1325314808 2019-01-03 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew performing a final pressure test on kill plugs in the 1H well. This well had two kill plugs set in it on 12/14/18 at 8029 and 7879. Plugs passed a negitive and a positive pressure test. Halliburton will be moving off of location starting today. Surface and intermediate annular pressures are 100 and 250 respectively.
1614025061 2019-05-08 Preliminary Restoration No Violations On the day of my inspection I visiting pad at the request of Adam Schroeder on behalf of Mr. Humpreys. Mr. Humpreys was concerned with no production on the 9H well which was due to shutting it in to drill the 1H, 3H, 5H, and 7H wells. Email was sent to Adam Schroeder informing him the well in back in production. This well is currently producing off of natural reservoir pressure. Production equipment includes five GPUs, five sand seperators, two line heaters, one combustor, three compressors, two vapor recovery towers, and eight 500 barrel steel tanks, within their own containment. The 13.375x9.625 annular pressure was 2 psi and the 9.625x5.5 annular pressure is 1605 psi. Burst pressure on the 13.375 casing is 3090 psi. Spoke with lease operator onsite and he said he would get the annular pressure blown down. Pad is stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
1787118301 2018-12-04 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. They were just finishing up stimulating stage 25/67. Plug was set at 14779, with bottom perforation at 14770 and top perforation at 14649. They pumped 24 barrels of 7.5% HCl acid, 4710 clean fluid, and 245270 lbs of sand (60070 lbs of 100 mesh sand and 185200 lbs of 40/70 mesh sand). Breakdown pressure was 6667 at 11 barrels per minute (BPM). Maximum rate was 86 BPM and average rate was 84 BPM. Maximum pressure was 11754 and average pressure was 10159. Initial shut in pressure (ISIP) was 4860 and 5 minute ISIP was 4560.
