0032500740 2018-07-26 Completion Testing No Violations On the day of my inspection I witnessed that the frac operator has moved off of the pad. Aboveground storage tank and 12 frac tanks remain onsite. Well connect is entering through the southern edge of the pad. No personnel onsite but fence is down and ground is excavated. Production equipment includes two 400 barrel steel tank inside containment, four sand seperators, and four GPUs. Wellhead gauges read 0 psi on surface, intermediate, and production casing. This well has not been drilled out or flowed back. Wellpad outslopes are stable and vegetated execpt for well connect disturbance.
0041533425 2018-08-20 Completion Testing No Violations On the day of my inspection I witnessed Falcon EFS flowback services onsite rigging up desanding units. ID to company man Brian Waid. Two wells are on location. These wells are set up to produce directly out of the 5.5 inch diameter production casing, on each side of the wellhead. There are six desanders onsite. Production equipment includes four line heaters, four sand seperators, four GPUs, and two 400 barrel steel tanks in their own containment.
0175134140 2018-05-14 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed patterson 580 reach total depth of their 8.75 inch diameter air section at 8077 feet. They are running a soap sweep then coming out of hole to load hole, run cement bond long on 9.625 inch diameter casing, and pick up direction tools.
0317817064 2018-12-31 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain two properly labeled wellheads. These wells are set up to produce directly out of the 5.5 inch diameter production casing, on each side of the wellhead, and are doing so on natural reservior pressure. There are five desanders onsite left over from flow back. They are dissconected from wells. Production equipment includes three line heaters (not plumbed in), four sand seperators, four GPUs, and two 400 barrel steel tanks in their own containment. Surface annular pressure is 0 and intermediate annular is 350. Site meets requirements of 1509.072A for preliminary restoration.

0615305110 2018-03-30 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 230 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. Cement was displaced with 39 barrels of fresh water. 18 barrels of cement returned to surface and then fell back. Top out job scheduled 4/2/18. Express cement supervisor Brian Sparks and David Newman CHK superintenent both onsite.
0677055097 2019-05-14 Work Over Reconditioning No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. During our inspection Conserv Inc. was onsite swabbing the 1H well and this well is next. Company representative Bob Vandermulen said that Stone Rig #15 ran 8690 feet of 2.875 inch diameter tubing in the well over the weeekend. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 480 psi.
1076427794 2018-06-20 Completion Testing No Violations On the day of my inspection I found this pad to contain two wellheads equipped with frac stacks. Two other frac stacks are onsite not on wells. No frac equipment onsite other than twelve frac tanks. Production equipment includes four sand seperators, four GPUs, and its gauges show no pressure. Well has not been stimulated.

Pipeline contractor appears to be restoring pad berm from pipeline connection. Several guys on location building flowline. Surface and intermediate annular pressures are zero.

1602091965 2018-07-06 Completion Testing No Violations On the day of my inspection I witnessed BJ services was rigged up on this well and permit 21543. They are preparing to frack stage 18. Top of perforation is at 18605 and bottom perforation is at 18765. There are five guns with 6 shots for a total of 30 perforations. Spacing is 40 feet apart. Surface and intermediate annualr pressures are zero. Fresh water aboveground storage tank (AST) on location. Average rate of stage was 78.6 barrels per minute. Average pressure was 9630 psi. Breakdown pressure was 8478 psi. 230510 lbs of 40/70 mesh sand pumped along with 99079 lbs of 100 mesh sand. Initial shut in pressure was 6376 psi. 72 barrels of 15% HCL.
1773662640 2018-07-16 Completion Testing No Violations On the day of my inspection I witnessed BJs frac crew on location. Surface annular pressure is 0 and intermediate annular pressure is 0. Stage 50 perforations are from 12365 to 12204. Plug is at 12385. Stage 50 is next planned stage and they will pump 100000 lbs of 100 mesh sand and 230000 lbs of 40/70 mesh sand.
