Cement was circulated to surface and no problems encountered with cementing. Pit for drilling fluid ripped during drilling. Well was drilled with fluid rotary rig. Drilling mud was contained in tank on site. Shaker removed drill cuttings from mud. Only fresh water was encountered and discharged into pit. The ground was clay and no tiles were noted in the field. Discussed pit liner with Dave. No salt water may be discharged into pit unless the pit liner is replaced.

An ID sign needs to be posted at the well. Final restoration needs to be completed. Spoke to local operator Tony who indicated that the well yet needs to be serviced and water in pit may be needed. ID sign has been ordered. Water will be pumped from pit, pit backfilled and final restoration completed.
1175192122 2007-03-29 Production Wells No Violations Well was drilled in October 2006 and is in production. ORC 1509.072 requires that pit used during drilling must be backfilled within 5 months. Further area around well not used for production must be properly graded, resoiled and planted within 9 months of drilling. Pit must be backfilled and graded immediately unless a written request and reason to leave pit open are submitted. Resoiling, grading and seeding must be completed by June. If access road is to remain permanent, a signed, written request must be obtained from the landowner. Also there is no ID at the well. ID sign must be posted immediately.

1426005450 2015-02-11 Production Wells Violations Noted At the time of the inspection the well was still shut in and has not produced since the previous inspection on 1-31-2013. The well is equipped with a pumping unit and is capable of production but no Production Records have been filed since 2006. The well is leaking gas where the tubing enters the wellhead. A LEL reading of 26 and a Methane reading of .60 was detected at the wellhead. No Hydrogen Sulfide was detected but this is a known Hydrogen Sulfide area so precaution should be taken during any service or plugging work. There has been no response from OGE Energy to a previous Compliance Notice #124359249 issued on 9-8-2011. This well is hereby ordered to be plugged and the land restored by 9-11-2015.

1618290140 2018-01-25 Production Wells No Violations FOLLOWUP ON NOV # 124359249 DATED 8/03/2011 and NOV # 1426005450 DATED 09/11/2015 Well consist of 7 surface casing, 2 3/8 tubing with valve shut in. Gauges are present at wellhead but do not appear to be reliable. There is a pump jack connected to wellhead but without a motor. No storage tanks on site. There is a 55 gallon barrel that is approximately 3/4 full of an oily substance. Slight odor of gas around well but no readings were obtained with gas detector equipment. Stone driveway is accessible from to well. No identification is posted at well and well appears to be incapable of production at the present time.
1808849200 2006-10-17 Production Wells No Violations Operator cemented 7 inch casing late Monday night. Spoke to operator Dave Buckholder about cement job and received copy of cement ticket. Cement was circulated to surface and no problems were encountered during cementing. Cement dropped 20-30 feet from surface by morning. Cement will be added from surface to top off annular space. Salt water used for drilling was contained in tank on site. Drill cuttings were removed with shaker. No brine was encountered during drilling and no salt water was discharged into pit.
1969491983 2013-01-31 Production Wells No Violations At the time of the inspection the well was shut. There was 0 psi on the annulus gauge and no gauges on the production lines. There were no signs of any leaks. The corrective action from the Compliance Notice issued in 2011 has not been completed. The inspection was done at the request of Eric Vendel from The Ohio Division of Oil and Gas.
