0396478761 2007-04-12 Production Wells No Violations An inspection of this well was requested by the U.S. Forest Service, Wayne National. Gary Cravens, Wayne National Forest, assisted with the inspection of this well. During the inspection, it was noted that the well is idle. The well was found to contain a pump jack with an engine. The polish rod showed signs of non-production. A flowline was observed but found not connected. No tanks were observed around the location. The well did not contain identification. Wayne National Forest believes that this well is the HVP 143 and may be owned by Valley Enterprises. They state that Valley Enterprises may have purchased this well from C&M Oil and Gas and failed to record the assignment. A new Lat & Long was obtained from this inspection. At the time of the inspection, the location of the original x, y, coordinates appear to be off. Research will be conducted to ensure correct permit number is assigned.
0727788768 2009-10-15 Production Wells No Violations Received notification of another potential oil spill from Northwood Energy well. Note: Rainfall event yesterday. Arrived at location and observed approx 4 of fresh oil in back half of earthen dike around 100 bbl tank with no knockout. The dike measured approx 15x 10 x1-2 (height). Oily ground surface throughout dike. A section of the dike has been trenched to allow discharge over hillside. The impacted area measured approximately 50 x 5. Oil sheen on surface of ground with organic material (leaves). Dead trees/vegetation below but may not be related. The source of release is from an opened valve on 100 bbl tank. Approximate release of oil at this time is 5 bbl. Recommended locks, bull plugs and cleanup procedures to remedy contamination and pollution. Checked Little Monday Creek to North and Northeast of location (approx 1000-1500 away), Sand Run Creek to Southeast of location (along Dawley Road to Jacobs Road). No oil observed in waters of the state. Northwood Energy believes this incident is related to vandalism.
1226685934 2008-05-01 Production Wells No Violations An inspection was conducted with Mr. Bob Barr. Mr. Barr was shown the locaiton of this well. During the inspection, it was noted that the status of the well has not changed. The well is still idle and missing identification. At this time, the well ownership remains in question.
1300840909 2008-12-17 Administrative Inspection No Violations An administrative inspection revealed that this well has now been transferred to Northwood Energy.
1459445262 2009-10-16 Production Wells No Violations A follow-up inspection was conducted with South Region Oil & Gas Supervisor, Mr. Robert Stonerock and Wayne National Forest representatives Mr. Tommy McDonald and Mr. Gary Craven. During the inspection, it was noted that bioremediation efforts have been applied to the areas impacted by spill.
