0538935298 2015-07-30 Orphan No Violations My inspection today was with Barry Bowers of Huffman-Bowers Co. who will be plugging this well. We had to walk back to the well due to a corn crop and the access road washed out. We found the well, very overgrown with weeds. The well was still leaking oil on the ground and gas to the air. Barry assessed the well and will provide an estimate.
0811678346 2016-05-31 Orphan No Violations Orphan well status check inspection with DOGRM Geologist Jake Glascock. Upon inspection, it was observed that the well is still leaking oil and gas out of the production string. No oil was observed in the stream due to low flow conditions. The well is currently overgrown with seasonal vegetation. The farm field through which the well is accessed appears to have been planted recently. Photos were taken.
0942009178 2018-04-06 Orphan No Violations Orphan Well Status Inspection: Upon inspection, it is observed that this well is in the same condition as previous inspections. The well consists of 8.63 diameter casing and 6.63 diameter casing which are both open to the atmosphere and leak oil, gas, and brine to the environment. Photos were taken of the well and access route. This well is included in the Jackson #2 Orphan Well Plugging Project which is scheduled to be bid out in July 2018.

1108176431 2005-06-03 Production Wells No Violations Well supposedly plugged; creek has meandered to where well is now in the creek bed. It is leaking oil between the conductor and surface casing and bubbling gas out of surface casing.
1192468228 2007-10-11 Orphan No Violations Well is located in stream bed. Currently the stream is not contacting the well due to dry weather and low flows. At flood levels the well will be under water. The well is leaking oil and brine into the sandy soils of the stream bed. The well is also leaking some gas from the surface casing. The surface casing is 6 5/8 and is open with oil and brine flowing out of the top, gas bubbles can be seen also. Conductor is 8 the outer lip of the 8 is bent over in some spots. Oil and brine are flowing down the 8 and onto the ground. To access the well you turn off of 776 (at GPS point N38.96967 W082.79556) into a corn field and follow the stream channel to the well site. A barbed wire fence will have to be taken down at the well site. 6-10 trees will have to be taken down to plug the well. Some site preparation will have to be done to allow the rig to get to the well. The edge of the stream bank is not stable enough for a rig to access the well. Plugging should occur during dry weather to reduce damage to site and make plug job more efficient.
1267833044 2015-09-29 Orphan No Violations My inspection today was to show with Joe Fredle, the Federal EPA Inspector the site to see if it would qualify for Coast Guard funds. Joe says it will qualify, due to the amount of oil around the site and that it was located in the floodplain at creek level. Huffman Bowers has been given the SOW and is submitting an estimate for the work.
1268069218 2018-05-08 Orphan No Violations Orphan well status inspection, accompanied by DOGRM staff members Rob Lowe, Lucas Smith, Trisha Cartmell, and Nate Lane: The purpose of this inspection is to familiarize DOGRM technical sections with this well location so they can perform their individual assessments for the Jackson #2 Plugging Project. On-site conditions have not changed for this well since the last inspection.
1364578768 2013-03-29 Orphan No Violations At the request of Michele Miller inspected this well. Met Johnnie Sparks in the field before I started to walk to well. I had him sign another right of entry for this well. I found wellhead next to the McDowell Creek. Wellhead consists of a 8-7/8 inch surface casing rising 3.5-feet above the ground surface. Inside this a 6-inch casing rises another 0.5-foot. Nothing is above this top. Gas is bubbling out of the liquid in the 6-inch casing. The PhD6 gas meter detected no methane gas (CH4). Liquid over flowing this surface dripping into the annular space between the casings and then over the side of the 8-7/8 inch casing to the sandy surface below.

2077536974 2015-02-11 Orphan No Violations .My inspection this day, accompanied by Kip Thompson, Jackson County Inspector and Mr Sparks, was required to update the Orphan Well Program file. Access to the well is west off of State Route 776 at GPS: 38.96969, -82/79536. Travel west through the farm field and just to the north of the tree line/McDowell Creek and approximately 2200 to the well, which is situated approximately 25 east of the creek.Mr. Sparks would prefer we plug this well between November and April in order to prevent damage to his crops. The well is equipped with 8 5/8 and 6 5/8 diameter casings. The well is leaking brine and oil. There is a 5 x 5 area around the well saturated with oil. Approximately 3 of the well casing is exposed above ground level. This area is prone to flooding, however there was no oil in the creek this day. Pictures were taken, SEM is 181.
