1359388909 2013-01-24 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 20 conductor for the Brown 3H well. 120 of casing was set in a 26 hole. Allied cementing services were 3 hours late arriving on location due to road conditions. The hole was cemented with 50% excess but no cement returns were seen. The cement was later tagged at 35 the following mourning and the backside of the conductor was grouted with ready mix cement. Nomac 38 is coming to drill this well when down on the Booth pad.
1359483668 2013-01-29 Drill / Deepen / Reopen No Violations I was onsite to check the grouting of the conductor on this well. The conductor was grouted and the basement is currently 7 feet deep. A crew is onsite setting matting on location and placing Greatplains frack tanks. Nomac 38 is running casing and will be moving on location after cementing the production casing.
1360702287 2013-02-11 Drill / Deepen / Reopen No Violations I was on location to pick up the paperwork for the surface and intermediate casing cement jobs. Both jobs went as planned and returned cement to surface. Nomac 38 had an issue with their blowout preventer freezing up internally before install, but the issue was taken care of and it passed all tests. The rig is currently mist drilling at 6300 feet.

1410822906 2016-03-10 Production Wells No Violations My inspection today found the access road gate locked and the access road in good repair. The pad has been fenced but parts of the fence on the south side of the pad have been removed for access to a major slip repair underway on that pad slope. I found the #3H well in production. Concrete barriers were in place around the wellhead. Well identification number was found on the well and on signage at entrance to access road. The equipment on the pad consists of the following:

2 400 bbl. production tanks contained in a metal dike. 4 400 bbl. production tanks not in containment near gate, not in use. 2 Producing wells. 2 Upright Sand Separators. 2 GPUs.

1482846477 2016-12-27 Production Wells No Violations Today I am onsite to check the state of the remediated western slope of the well pad for this permit number after a number of days with heavy precipitation. The slope has many deep channels carven into it as well as a few small landside areas. The drain at the base of the slope appears to be partially filled in with sediment that bypassed the filter socks and silt fences. I notified the Engineer Paul Logan of the situation.

1698876749 2013-04-01 Completion Testing No Violations I was onsite to check the status of the hydraulic fracturing of this well. The frac job was completed and equipment has been removed from the location. Currently the well has an assembled christmas tree wellhead on it and has been recently painted. There are three Great Plains frac tanks on location and the 10H wellhead is still shut in. All production complete is still onsite and appears will be put into production when the well is flowed back. Containment is still in place and thee are no violations at this time.
