0035237882 | 2013-06-10 | Not Drilled | No Violations | I was at this location to find the access road for the drilling pad that is going to be built in the near future for Chesapeake. A presite meeting is set for Thursday June 13th for this location. 2 wells have been permitted for this pad location but no work can be completed on the pad until the presite is completed. It appears that the pad will be built in a large field that is currently just a non agriculture grass field. |

0398648272 | 2013-11-06 | Drill / Deepen / Reopen | No Violations | I was onsite to pick up the cement paperboard from intermediate casing and check the status of drilling. Currently the rig is drilling the horizontal of their well at 11280. At the time they were performing a cleanup cycle on the well. |

0446085917 | 2013-07-29 | Not Drilled | No Violations | I was at this location to check on the construction of the well pad. So far only parts of the pad have been completed, Currently they are removing topsoil in an attempt to level the pad for further construction. There are 2 wells permitted on the location currently. There are no violations at this time. |
1261083009 | 2013-12-09 | Drill / Deepen / Reopen | No Violations | This well has been drilled and capped off with a 5000psi rated wellhead cap. There is no equipment left on location but the rig mats are still present. Once the location is clear it will be prepared for the completions crew to come on a later time. Erosions controls are established and are in good shape for the upcoming winter. There are no violations at this time. |

1306423795 | 2013-08-09 | Not Drilled | No Violations | I was onsite to check the status of site construction for this pad. Currently the entire location is graded and set but no gravel has been put down yet. The drainage controls are also in the point of construction but are not completed yet. There are no violations at this time. |
1338706505 | 2017-07-24 | Production Wells | No Violations
1371827036 | 2013-06-17 | Not Drilled | No Violations | I was onsite to conduct a presite for this pad location with Cody Theimer from Chesapeake. The location is in a large field. Currently only the wellstake is on location. They stated that construction will begin on the well pad in the fall when Chesapeake intends to bring a drilling rig to Jefferson County. |
1377540252 | 2013-08-21 | Not Drilled | No Violations | I was on location to check the status of wellsite construction and erosion control placement. Currently rock has been laid over the entire pad and the erosion controls have been installed. Seed has not yet began to grow due to the lack of rain and the recent placement but it should be established by the time a rig comes onto location. The 1H well is spiked out currently so it appears only one well will be drilled on this location at this time. There are no violations at this time. |
1377890224 | 2013-08-30 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 30 inch conductor casing. My inspection revealed that Express Energy Services has drilled a 36 inch hole to a total depth of 120 feet and ran 117 feet (120 feet total with 3 feet sticking up above ground level) of 30 inch casing into the well bore. Allied cemented the casing with 300 sacks of Class A (2% CaCl and 1/2 pound per sack floseal). The cement had a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. Allied pumped 64 barrels of cement slurry, 81 barrels of freshwater displacement, and got 15 good barrels of cement returns. My inspection revealed no violations at this time. |
1382719401 | 2013-10-24 | Drill / Deepen / Reopen | No Violations | I was on location to check the status of Nomac 77 rigging up on this well. They expect to spud the well tomorrow night. They are going to drill a 24 surface hole to 400. 150 above the proposed coal mine void. |
1382922544 | 2013-10-27 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 20 surface casing that was set in a 400 24 hole. It took the rig 12 hours to wash down the casing before the job was pumped. The job commenced but the cement head leaked following the placement of the 20 wooden plug. After the cement head was tightened the job continued and good cement was returned to surface weighing in at 15ppg. Nomac 77 is expected to hit a mine at 550 for which a mine string will be required. |
1383680886 | 2013-10-27 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 20 surface casing set to 400. The job went as planned with the exception of the cement head leaking and the need for the stopping of the job and retightening the head. In total 53 barrels of cement were circulated to surface weighing 15.3 ppg. |
1391970668 | 2013-10-31 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 9 5/8 intermediate casing 2 set to 1900. The job went as planned and cement was circulated to surface weighing 15.4 ppg after being mud scaled by the Schlumberger operator indicating a good cement job. They will continue to drill to their total depth following their 8 hour wait in cement to harden before blowing the hole clean. See casing ticket report for further details. There are no violations at this time. |
1402422382 | 2014-06-10 | Preliminary Restoration | No Violations | I was on location to pass preliminary restoration and determine the production status for this well. I was contacted by Chesapeake stating that this well has been put into production. The well contains the Christmas tree valve assembly and proper annulus gauges. Production equipment for the well has been installed and includes separators and two 400 bbl production tanks surrounded by a steel dike. This well will begin production with temporary production equipment, used to collect flow back from the frac job, tomorrow. This will occur for several days. Erosion controls are present on location and vegetation has been established. The location is fenced and has proper I.D. signage on the tank, wellhead and lease road. Preliminary restoration has been passed at this time. |
1453833661 | 2016-01-26 | Production Wells | No Violations | On my inspection today I found two wells in production with Christmas tree valve assemblies with tubing and annulus gauges. Production equipment for the site includes two separators and two 400 barrel production tanks that are located within properly sized steel containment. The earthen berm surrounding the pad is in place and vegetation has been established on all slopes. The location is fenced and has proper I.D. signage on the tank, wellheads and access road entrance. |
1457045149 | 2016-03-03 | Production Wells | No Violations | My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #1H well in production. Concrete barriers were in place around the wellhead. Well identification number was found on the well and on signage at entrance to access road. The equipment on the pad consists of the following:
2 400 bbl. production tanks contained in aluminum dike. 2 Producing wells. 2 Upright Sand Separators. 2 GPUs. |

1620319645 | 2016-11-28 | Production Wells | No Violations
1820333476 | 2013-07-11 | Administrative Inspection | No Violations | I was contacted by Cody Theimer of Chesapeake to find out the status of the presite for this well pad location. The presite paperwork was misplaced and has been resubmitted to Columbus so that it can be forwarded to the companies regulatory division allowing them to start well pad construction. |
2002892865 | 2013-11-28 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 13 3/8 intermediate 1 casing set to 603. The job went as planned and in total 28 barrels of cement were circulated to surface and weighed 15.1 ppg (weighed using Schlumberger mud scale) in the cellar. This string was a mine string in which the mine was not hit. They still set the casing so that they could set their blowout preventer. |
