0213202512 2013-11-20 Drill / Deepen / Reopen No Violations The rig is currently drilling at 1450 and has been drilling with not returns. They have been using large amounts of cedar fiber and cotton seed hulls without being able to establish returns. The 12.5 hole has been drilled on fluid and several pallets of loss circulation material has been used at this time. They currently are ordering on location 2500 barrels of water so that they can TD the well. They are planning on using a 2 stage cementing tool for this job due to a lost flow zone from 530 to 570. Cement will be circulated to the loss flow zone and the DV activated to cement to surface .
0244439895 2014-06-10 Preliminary Restoration No Violations I was on location to pass preliminary restoration and determine the production status for this well. I was contacted by Chesapeake stating that this well has been put into production. The well contains the Christmas tree valve assembly and proper annulus gauges. Production equipment for the well has been installed and includes separators and two 400 bbl production tanks surrounded by a steel dike. This well is currently being produced with temporary production equipment that is being used to collect flow back from the frac job. This will occur for one more day then the 1H will be flowed back. Erosion controls are present on location and vegetation has been established. The location is fenced and has proper I.D. signage on the tank, wellhead and lease road. Preliminary restoration has been passed at this time.
0245787665 2013-11-22 Casing String Witnessed No Violations I was on location for the 9 5/8 intermediate cementing. This was a 2 stage cement job to isolate a loss flow zone from 572 to 530. A packer was set at 525 and a DV tool at 520. Cement was circulated to the to the loss flow zone at 572 with excess. Returns were not established. Therefore the DV was used to circulate cement to surface from 525up. This cement job should isolate the loss flow zones and cover up the shoe track from the previous casing string that was degraded and had a formation integrity test failure. The job consisted of 725 sacks of class A cement at 15.6ppg. 22 bbls of cement were returned to surface .
0253659889 2013-06-10 Not Drilled No Violations I was at this location to find the access road for the drilling pad that is going to be built in the near future for Chesapeake. A presite meeting is set for Thursday June 13th for this location. 2 wells have been permitted for this pad location but no work can be completed on the pad until the presite is completed. It appears that the pad will be built in a large field that is currently just a non agriculture grass field.
0396812716 2013-12-09 Drill / Deepen / Reopen No Violations This well has been drilled and capped off with a 5000psi rated wellhead cap. There is no equipment left on location but the rig mats are still present. Once the location is clear it will be prepared for the completions crew to come on a later time. Erosions controls are established and are in good shape for the upcoming winter. There are no violations at this time.
0486832339 2013-06-17 Not Drilled No Violations I was onsite to conduct a presite for this pad location with Cody Theimer from Chesapeake. The location is in a large field. Currently only the wellstake is on location. They stated that construction will begin on the well pad in the fall when Chesapeake intends to bring a drilling rig to Jefferson County.
0965783968 2013-11-12 Casing String Witnessed No Violations I was on location to witness the cementing of the 20 surface casing inside of a 26 hole. There were some issues with running the casing due to the size of it, this is because the Nomac Pike Rigs are not designed to run 20casing. They also had to run a pup joint which forced the crews to have to be in saddles raised to put the cement head on. The pup joint then had a bad collar and leaked twice before the rig was able to even begin to run the cement job. Tongs had to be brought onto location to remove the pup joint and place the cement head on the lower joint. The cement head fit well and did not leak. I had to leave the location and was informed by the company man (Tim Allen) and the Schlumberger cement report that the job was pumped and good cement was returned to surface per the Schlumberger log. They will be drilling another 180 roughly before they hit the proposed mine void at a possible depth of 520 approximately.

1385134059 2013-11-19 Administrative Inspection No Violations Nomac 77 called and asked for allowance to shorten their intermediate casing set point from 2000 to 1685. This would be set 200 below the Berea sandstone. I talked with inspector Farnsworth and deemed setting the casing shorter would not be am issue.
1436192666 2016-03-03 Production Wells No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #6H well in production. Concrete barriers were in place around the wellhead. Well identification number was found on the well and on signage at entrance to access road. The equipment on the pad consists of the following:

2 400 bbl. production tanks contained in aluminum dike. 2 Producing wells. 2 Upright Sand Separators. 2 GPUs.
