1111508205 2018-03-26 Preliminary Restoration No Violations On the day of my inspection I found this pad containing two properly labeled wells. Surface and intermediate annular pressures read 0 and 530 psi, respectively. Intermediate annular casing is plumbed with a motor valve and tied into a buried flow line. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tanks, within their own steel containment. Production records are up to date. Pad is stable and vegetated. Site meets requirements on ORC 1509.072A for preliminary restoration
1142950418 2016-08-02 Casing String Witnessed No Violations I am on site today to witness the cementing of the conductor casing. Express energy services drilled a 24 inch diameter hole to a depth of 125 feet and then proceed to run 123 feet of 20 inch diameter conductor casing. Express Energy Services is the cement contractor on site. Expressed pumped 230 sacks of class A cement and then displaced with 39 barrels of fresh water. Over the duration of the job I witnessed good cement returns to surface level. Approximately 2 foot of cement was left in the cellar.

1479308213 2016-11-16 Drill / Deepen / Reopen No Violations I am onsite today to inspect the well pad during drilling operations. Nomac 80 is currently drilling on fluid at 7956 feet and preparing to start their curve. Upon inspection of the secondary containment onsite I found 2 areas with torn liners. I also found drums of cleaning solvents outside of secondary containment. I spoke with the company man Jackie about my findings. He contacted Tetra Tech about the torn containment and said it would be repaired today. He also had the drums moved into the storage area with secondary containment.
1479825633 2016-11-22 Drill / Deepen / Reopen No Violations On the date of my inspection I found Nomac 80 rigging down. Grant Trucking is currently hauling pieces of the rig over to the Williamson Pad. Currently approximately 20 percent of the rig has been moved and they are currently working on steam cleaning portions of the rig while disassembling the mast portion of the rig. The initial drilling operations for both wells on the pad are complete.
1920781111 2016-08-08 Drill / Deepen / Reopen No Violations I am onsite today to verify the cement level in the tin horn after setting the conductor. On 8/3/2016, I noticed that the cement in the annulus around the conductor had fallen to 2 feet below the ground level within the tin horn. Cement has since been added and is now approximately 78 inches below the top edge of the tin horn, leaving approximately 18 inches of cement as a seal in the bottom of the tin horn.
