1351131628 2017-03-01 Plug / Plug Back No Violations On the day of my inspection I found that R&J Well Service was not plugging. No one was present at the well site. The rods and tubing had been removed from the well and transported off site. I called Randy Moran and left a message to update me on his intended schedule.
1475606812 2016-10-04 Production Wells Violations Noted This well has had the pump jack removed and lines have been disconnected and the well is incapable of production and shall be plugged in 90 days from todays date 10/4/2016. The well has no identification posted. Valley Petroleum Management shall submit a written plugging schedule to Mark McCombs at 116 East High Street Mt. Vernon Ohio 43050-3402 within 30 days, per OAC 1501:9-11-05(B)(B) Prior to commencement of plugging operations for a dry hole or lost hole, the owner, or his authorized agent, shall furnish the inspector with a record of the depth of the top and the base of formations penetrated during the drilling of the well, the depths where oil, gas or water were encountered, and a record of any casing present in the well. If reasonably available, the information furnished shall include a copy of the geophysical, electrical, or mechanical log and/ or the drilling contractors report. Based on the furnished information the inspector shall approve a plugging program in accordance with rules 1501:9-11-01 to 1501-9-11-13 of the Administrative Code and plug emplacement methods and plugging materials approved by the chief. When no records are available, the owner, or his agent, shall make every reasonable effort to obtain sufficient formation and casing data on the well, to assist the inspector in determining the plugging procedure. Exceptions may be granted at the discretion of the chief., and plug the well within 180 days. Valley Petroleum Management will be notified if the plugging schedule is approved or denied.
1483988628 2017-01-09 Production Wells Violations Noted Talked with Denny Dugan from Valley Petroleum Management to discuss when he intends to plug this well. He stated that R&J well service was moving in to plug the well and satisfy this compliance notice.

1488570361 2017-03-03 Plug / Plug Back No Violations On the day of my inspection Denny Dugan of Valley Petroleum delivered 20 tons of clay from Logan Clay. R&J Well Service washed 2 tons of clay down the 5.5 inch casing and went in to rip. Randy Moran got a telegraph at a depth of 2500 feet and parted the casing there. Randy pulled 20 joints and shut down for the evening.
1488634663 2017-03-04 Plug / Plug Back No Violations On the day of my inspection R&J Well Service washed another 6 tons of clay in the well, for a total of 8 tons in the bottom plug. They also started to pull the rest of the 5.5 inch casing from the well.
1488989931 2017-03-08 Plug / Plug Back No Violations On the day of my inspection Randy Moran ran in with a pre-cast cement plug to find the 7 inch seat. There were no records of 7 inch casing ever being ran when this well was drilled. Denny Dugan claimed that they did run 7 inch casing. Randy also said that the hole would have been reduced at some point below the Berea, regardless of whether 7 inch casing was used. The pre-cast set down at a depth of 994 feet. Randy set the pre-cast and then bailed the hole dry. Once the hole was bailed dry down to the pre-cast they washed clay in until they got up into the surface casing. The surface casing is cemented, and will be left in the well. Randy wanted to fill the casing with dry clay. They shoveled dry in the surface casing up to 300 feet and shut down for the night.
1489072409 2017-03-09 Plug / Plug Back No Violations On the day of my inspection R&J Drilling shoveled the rest of the clay into the well, filling the surface casing to within 5 feet of ground level. There is a total of 12 tons of clay from the pre-cast at 994 feet to surface.
