At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated battery in separate board-on-board enclosures. The enclosure gates were locked. The load-line valves were locked and secure. The secondary containment was intact and free from signs of spills, leaks, or other problems. The gravel-paved access road was in good condition. I noted that there was some leaked oil under the 2-inch hammer union at the wellhead and I notified the well owner.

Spoke with Tim Reilly regarding fencing the perimeter of the site, he stated he would have it fenced by the end of the day. Rig set 1 jt new R-3 32# 11.75 casing to 42.3 ft. Casing will be cut down to approx 32. Hole is 15 TD 39. Hole is static. Universal on-site to cement conductor, broke circulation and ran 50 sks Class A 3% CaCl and 1/2lb flake. Cement circulated to surface, displaced cement to 27 using 3.3 bbl fresh water. 6 bbl good cement returned to surface.

Set 8.625 new R-3, 24# casing at 209. Circulated on hole for one hour waiting for cement to arrive. Universal broke circulation and pumped 110 sks GasBlock cement and displaced to 199 using 12 bbl fresh water. 12 bbl good cement returned and held at surface.

The spacing is in accordance with the rule, and the valving and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a mechanical plunger lift and is located 75 feet north of the TB. The tank battery dike is adequate. Locks and ID are good. No violations noted.
1461774623 2016-04-27 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and battery are located via a dedicated, unpaved access road which enters from White Road. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system and tank battery in separate board-on-board enclosures. Enclosure gates were locked and secure. The tank load-line valves were locked and secure. Tank lids were closed and secure. Secondary containment was present and intact. I observed the following safety/maintenance deficiencies: The surface casing nipple is exposed but not gauged as required by permit condition. The ID signage identifies John D. Oil & Gas as the current owner which conflicts with current record keeping indicating Osair, Inc. The owner must complete the following by May 27, 2016: Properly gauge the surface casing nipple, update signage to reflect the current owner. Contact Inspector Stephen Tompkins at 330-907-4647 with question regarding this inspection.

The wellhead and battery are located via a dedicated lease road that runs north from White Road. A portion of the lease road is shared with a telecommunications tower access road. The wellhead is comprised of a plunger lift system and battery (2 100-barrel steel tanks) in separate board-on-board enclosures. Fittings and connections at the wellhead and battery are secure. Load lines are secure and plugged. 12 to 15 inches of snow cover obscures some surficial features and areas. The area surrounding the well and battery is mostly wooded, and lies adjacent to I-271.

1994366096 2017-04-17 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for improper ID and missing nipple and gauge at the surface casing which was a permit condition at the time of the issuance of the original drilling permit. At the time of this inspection, I observed the following: the ID had been updated to reflect the change in ownership, although the ID signage remains missing information required under OAC 1501:9-9-05(E)(1). The surface casing nipple has been properly gauged as required under the permit condition. The load-line valves were locked and secure. The enclosure gates were locked. The areas surrounding the wellhead and battery were free from signs of impact or observable degradation resulting from the on-going production activity. I have attempted to contact the owner regarding the need for upgrade to the ID signage and, at the time of the filing of the report, have not received a response.
