0035071826 2013-11-06 Production Wells Violations Noted There is no well identification at the wellhead. According to 1501:9-9-05(A)10/1509:23 the wellhead should have attached a sign with at least the following information: current well owner, lease name, well number, permit number, county, and an emergency telephone number. The owner shall install a sign at the wellhead with at least the information listed above. The well is supplying domestic gas supply to a farmhouse west of wells location. No one was at home during my inspection. I will try to contact the homeowner to determine if they are the current owner of this well. There is no tank battery associated with this well.

1197339097 2017-11-15 Production Wells No Violations On the day of this inspection, I am doing a follow up to NOV#-35071826. The gas well status has not changed, there is still no identification posted at the wellhead. I gave Mr. Hosey the landowner a call and left a message and reminded him about the identification and gave him directions and information needed for filling out the identification for gas well.
1225016624 2018-08-22 Production Wells No Violations The purpose of my inspection today was to determine if the well had been brought into compliance. There is no phone number listed in RBDMS for a contact. I spoke with a neighbor and they said a Mr. Hosey owns the well. I spoke with Mr. Hosey and he admitted he owned the well and is using the well for domestic use. He was leaving when I arrived and I requested he call me at his earliest convenience. The well lacks a legible identification. The well consists of a conductor casing with 5.5 inch diameter casing. The 5.5 inch diameter casing is swaged to 2 inch diameter. There is 2 inch diameter production casing going into the ground. The valve on the 2 inch diameter production casing is in the open position. New pictures and coordinates were taken during my inspection.
1840510228 2018-09-17 Production Wells No Violations The reason for my inspection was to determine if the well was in compliance. The well remains in violation without a legible identification. I texted well owner Mr. Tom Hosey at 11:55 AM. I asked for him to contact me regarding placing a legible identification at the well and also getting the well registered in his name. I provided him contact information for division employee Kathryn Smith regarding domestic ownership.
