0105132312 2017-06-08 Production Wells No Violations Went down and still found 3-4 pockets of oil in the small stream. Spoke with the well tender and explained that would need attention. I suggested flushing the stream again and building another siphon dam near the source of the oil coming from the bank. He said that they already discussed that and were going to do it on Monday when the field supervisor came back from vacation. I put down about 5 diapers in different spots up the stream. I also spoke with them about some straw near another dike. They said they had a small release resulting from having the lines shut down because of this spill. They said they would remediate the area of about 4x10 feet on Monday when they have the track hoe here.
0123336469 2017-06-21 Production Wells No Violations Follow up inspection found that the creek was void of pools of oil today. The lower siphon dam was still in place and functioning correctly. The upper siphon dam was washed out, however the socks placed across the creek were still in place. This upper area is where the oil came out after going subsurface. There was no visible oil today.
0453574731 2017-06-01 Production Wells Violations Noted I responded to a release of 3-4 barrels of oil as estimated by Mike Carr field supervisor for Pillar Energy. I arrived at the location and they were using a pump and their water truck to suck oil out of the creek/ditch. I assessed the situation and went to a location that had cell service to give Mr. Scott King an update of my findings. I explained that the spill was from a flow line that was buried. The oil came to surface near the origin then traveled downslope and disappeared subsurface. The oil then came out of the ground just above the creek line. The oil traveled approximately 175 yards down the stream where it pooled up in front of a concrete spillway that was a culvert that crossed St.Rt. 775. I stayed until the siphon dam was installed and operational. Once the track hoe arrived the source was fixed then the siphon dam was installed. One 40 barrel load of fresh water was flushed down the creek.

1500387786 2017-07-18 Production Wells No Violations The spill has been cleaned up and site on top of hill where the oil went subsurface is starting to sprout vegetation. No oil or evidence of oil was found in the creek.
1968347600 2017-11-15 Production Wells No Violations On the day of this inspection, I found an oil and gas well equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing with a pumping unit. Identification was located at the wellhead and is legible.
