0804149408 2017-11-29 Orphan No Violations Orphan well initial inspection: Upon inspection, it is observed that this well is idle, incapable of production, and has been abandoned. The visible portion of the well consists of 5.5 diameter production casing with a reducing swage leading to a 2 diameter ball valve and another reducing swage leading to a 1 diameter ball valve and a pressure gauge. The pressure gauge is reading 20 psi. There is concrete present on the annulus of the 5.5 diameter casing at ground level. A previous inspection report indicates that there is conductor pipe present, but it is very corroded. There are no flow lines attached to the well and there is no surface equipment associated with the well. The well is not leaking oil, gas, or brine. There are no overhead utilites near the well location. The well is located in a residential area in Proctorville approximately 30 from a roadway and less than 100 from several houses and outbuildings. The landowners grandson states he is purchasing the house on the property from his grandmother via land contract, and he does not recall the well ever being produced either commercially or domestically. A voicemail was left for the landowner, Ms. Joyce Hazlett, in regard to the well and the potential for the well to be plugged through the Orphan Well Program. A right of entry form and business card was left with Ms. Hazletts grandson to give to her to fill out and return to the Divison. Photos were taken of the location.
