1483038919 2016-12-14 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Geologist Jake Glascock: Upon inspection, it is observed that this well is idle, incapable of production, and is likely orphan. The well consists of 10.5 outside diameter surface casing which is open to the atmosphere. No production casing or tubing is present in the well. There are no flow lines connected to the well or visible in the area. There are no tanks or other production equipment associated with the well. The well is located 15 from the banks of Symmes Creek and gets submersed in water when flooding occurs, allowing oil to flow into the creek. The fluid level in the 10.5 casing is 2 down and is primarily oil. Gas bubbles through the oil periodically. The well is in an open pasture area and is easily accessible via an existing road to P#087-20312 (also a potential orphan well). No trees or other vegetation need cleared to access the well. No overhead or buried utilites are located near the well location. A Plugging Application and Right of Entry Form was left with the landowner to fill out and return to the Division. The Site Evaluation Matrix score for the well is 192. Photos were taken.
