0100157223 2014-10-01 Administrative Inspection No Violations A phone call was placed at 8:46 AM to well owner Mr. Jim Seibert regarding non-compliance. Mr. Seibert stated that work should be finished by the end of next week.
0214947490 2014-04-11 Production Wells No Violations This inspection was conducted as a production well status check. The well was equipped with a pumping and motor. It was not pumping during inspection. An identification sign was laying on the ground near the well with the incorrect permit number. The tank battery consists of (2) 100 barrel tanks. One of the tanks is located near the well and the other is located at the bottom of the hill near Reform Rd. This well shares the tank battery with permit number 2541. Identification was not posted at the tank battery. It appears that water has been drained from the tank near the well over the hill in the past. The well owner, Mr. James Seibert, has been notified of inspection. Mr. Seibert was informed that this well will need to be placed under his bond with the Division and that proper identification shall be posted at the well and tanks. Mr. Seibert was also advised that it is illegal to drain brine water onto the ground and that the water shall be disposed of through a registered brine hauler with the State of Ohio. Photos were taken for documentation purposes.

Mr. Seibert said that he would work on getting well registered and will have his landowner post identification.
1326762106 2016-08-12 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. The tank battery was inspected. There was no change since previous inspection dated 6/16/2016. The identification was not correctly labeled with permit number 2541. The 100 barrel tank that is not in use has not been removed and the soil around that tank has not been cleaned up. The inspection revealed that brine is being drained onto the ground inside the dike from behind the 100 barrel tank where a 2 nipple fitting and valve was observed. There was no bull plug on the valve. The sample of soil was taken within 1 foot of the valve. The soil sample was tested with a High Range Quantab Titrator for Chlorides. This resulted in a 9.0 Quantab unit reading which is off the scale. An 8.6 Quantab unit reading on this particular scale equals 6,215 ppm for chlorides. I took 10 photos of the tank battery this date. Time of inspection: 12:20 PM. The tank battery is not in compliance.

1424893099 2015-02-12 Production Wells No Violations This inspection was conducted as a status check. I was accompanied by Jeff Fry. During the inspection we measured the dimensions of the tank battery dike. The dike dimensions measured approximately 170 barrels. The tank battery consists of (1) 100 barrel tank and (1) approximately 2000 gallon tank. The old 100 barrel tank is still on location. Identification has not been posted at the tank battery. The well owner, Mr. Seibert, has not registered well with the DOGRM. A phone call will be placed to Mr. Seibert regarding this inspection and the well status. Time of inspection: 11:40 AM. 1484929035 2017-01-19 Production Wells Violations Noted This inspection was conducted at 12:00 PM as a status check. I was accompanied by Inspector Justin Smith. The inspection revealed that a spill/release has ocurred at the tank battery. It appears that oil has run over the 100 barrel storage tank onto the ground. The dike may have contained the oil but it also appears that someone has taken buckets or shovels of the oil and dumped it in the weeds surrounding the tank battery location. Standing water inside the dike was tested with a Quantab titrator test strip for chlorides. This resulted in a 10.0 reading on the test strip which equals >6200 ppm chlorides on this test strip. The plastic brine storage tank within the dike did not have a bull plug in the valve on the front side of the tank. Brine may have been drained from this valve as there was an observed depression in the soil in front of the valve. The well owner will need to install a bull plug on the valve. Orange ribbon was placed around the tank battery location noting the extent of the oil that was dumped in the weeds. A ditch near the tank battery was observed for any impact that may have taken place due to the release. I did not observe a sheen or oil on the water. During the inspection the Emergency Response section was notified of this incident. A voicemail message requesting a return phone call was left for Mr. Seibert (614-620-3662) at 1:22 PM. At 5:00 PM I spoke to Mr. Seibert via phone conversation regarding this incident. Mr. Seibert was advised that clean-up would need to begin immediately. It was made clear that clean-up would consist of excavation of contaminated soil outside and within the dike. Excavated soil would need to be hauled to a landfill or land farmed where it could be remediated. Clean-up will be witnessed by an inspector. Mr. Seibert failed to notify ODNR in a timely manner of this incident in accordance with OAC 1501:9-8-02 Incident Notifications which took effect August 9, 2016.
