0085697438 2017-11-14 Production Wells No Violations An inspection of the tank battery revealed that the stockpiled soil has not been spread on the area that the tank was located. An email was sent to Mr. Seibert on 11-1-2017 asking if any work has begun at the tank battery. Mr. Seibert replied that day saying he would ask his landowner who is helping him with the clean up and he would get back with me. Mr. Seibert has not contacted me since. 0099140611 2014-06-05 Production Wells No Violations This inspection was conducted as a compliance follow-up. I was accompanied by Northwest Region Supervisor Will Ziegler and Jeff Fry. The well owner Mr. Jim Seibert and landowner Mr. Jack Trumbull met us at the tank located near the well (p#0387). A remediation plan was discussed with Mr. Seibert. Mr. Seibert agreed to build a new tank battery at the bottom of the hill and set both 100 barrel tanks there. The area near the well (p#0387) will be remediated in place with lime. Mr. Seibert will work on getting p#0387 registered and production reports sent in. He was advised to place proper identification at both wells and tank battery and to make sure that brine is being hauled through a registered brine hauler. Mr. Seibert was advised to keep both wells shut-in until he is in full compliance.
0102873477 2014-04-11 Production Wells No Violations This inspection was conducted as a production well status check. The well was equipped with a pumping unit and motor. It was not pumping. Identification was not posted at the well. The tank battery consists of (2) 100 barrel tanks. One of the tanks is located near the well and the other is located at the bottom of the hill near Reform Rd. This well shares the tank battery with permit number 0387. Identification was not posted at the tank battery. It appears that water has been drained from the tank near the well over the hill in the past. The well owner, Mr. James Seibert, has been notified of inspection. Mr. Seibert was informed that proper identification is required at the well and tank battery. Mr. Seibert was also advised that it is illegal to drain brine water onto the ground and that the water shall be disposed of through a registered brine hauler with the State of Ohio. Photos were taken for documentation purposes.
0158460912 2015-02-12 Production Wells No Violations This inspection was conducted as a status check. I was accompanied by Jeff Fry. During the inspection we measured the dimensions of the tank battery dike. The dike dimensions measured approximately 170 barrels. The tank battery consists of (1) 100 barrel tank and (1) approximately 2000 gallon tank. The old 100 barrel tank is still on location. Identification has not been posted at the tank battery. 0209475828 2017-02-27 Production Wells No Violations I returned to this well today to do a follow-up inspection on it. It appears the owner has started working on the location. There has been some contamination removed and placed on a blue tarp. I have taken pictures of my findings today for my supervisor.
0217583522 2014-05-28 Production Wells No Violations This inspection was conducted as a compliance follow-up. This well produces into a shared tank battery with permit number 0387. Identification was not posted at the tanks. The inspection revealed that brine appears to have been drained onto the ground from the 100 barrel tank near the well (p#0387). Stressed vegetation was observed around the well and tank location. Starting from the tank near the well to the tank at the bottom of the hill an area approximately 150 feet long by 10 feet wide has been extremely affected by brine kill. Several dead trees were observed in this area. The three inch valve on the tank near the well was in the open position with a three inch plug in it. A 1.25 plastic yellow line comes off the bottom of the tank and runs south approximately 75 feet and then runs over the hill towards the road (Reform Rd.). The yellow line is connected to a tee fitting that has a black plastic line coming off the middle outlet. It could not be determined if it is going to the tank at the bottom of the hill. The third outlet on the tee fitting had no valve and was open ended. It appears this has allowed brine to be drained onto the ground as there was a large area of stressed vegetation. The well operation is causing pollution and contamination. The well owner has failed to report production for 2013 on the well identified as the W & M Wills #1 (p# 0387). The well owner is also in violation for failure to register in regards to the W & M Wills #1 (p# 0387). Identification was not posted at the tanks. A phone call was placed to Mr. Jim Seibert at 10:42 AM and was informed of the inspection. Mr. Seibert stated that he was the owner of both wells on the Mills lease and he was advised to shut the wells in until further notice. A meeting will be set up with Mr. Seibert at the well to discuss well operations and a plan to resolve compliance issues. At this meeting it will be decided specifically what is required of Mr. Seibert in order for him to be in compliance a
0266322771 2017-07-10 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. Mr. Seibert was advised via email on 6/9/2017 to clean up oil contamination around the old 100 barrel tank that is no longer in use and clean the sides of the 100 barrel tank inside the dike. The inspection revealed that nothing has been cleaned up. Four photos were taken of the tank battery. 0817571469 2016-08-12 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. The tank battery was inspected. There was no change since previous inspection dated 6/16/2016. The identification was not correctly labeled with permit number 2541. The 100 barrel tank that is not in use has not been removed and the soil around that tank has not been cleaned up. The inspection revealed that brine is being drained onto the ground inside the dike from behind the 100 barrel tank where a 2 nipple fitting and valve was observed. There was no bull plug on the valve. The sample of soil was taken within 1 foot of the valve. The soil sample was tested with a High Range Quantab Titrator for Chlorides. This resulted in a 9.0 Quantab unit reading which is off the scale. An 8.6 Quantab unit reading on this particular scale equals 6,215 ppm for chlorides. I took 10 photos of the tank battery this date. Time of inspection: 12:20 PM. The tank battery is not in compliance.

All of the contaminated soil has been cleaned up besides what is left in front of the tank that needs removed. According to Mr. Seibert he has very little money to work with due to oil prices and the landowner has been pumping the well. I explained to Mr. Seibert that he could take the contaminated materials to an offsite location and treat the material. Mr. Seibert will submit a plan and timeframe on clean-up of contaminated materials first of next week.

1793909346 2017-01-19 Production Wells Violations Noted This inspection was conducted at 12:00 PM as a status check. I was accompanied by Inspector Justin Smith. The inspection revealed that a spill/release has ocurred at the tank battery. It appears that oil has run over the 100 barrel storage tank onto the ground. The dike may have contained the oil but it also appears that someone has taken buckets or shovels of the oil and dumped it in the weeds surrounding the tank battery location. Standing water inside the dike was tested with a Quantab titrator test strip for chlorides. This resulted in a 10.0 reading on the test strip which equals >6200 ppm chlorides on this test strip. The plastic brine storage tank within the dike did not have a bull plug in the valve on the front side of the tank. Brine may have been drained from this valve as there was an observed depression in the soil in front of the valve. The well owner will need to install a bull plug on the valve. Orange ribbon was placed around the tank battery location noting the extent of the oil that was dumped in the weeds. A ditch near the tank battery was observed for any impact that may have taken place due to the release. I did not observe a sheen or oil on the water. During the inspection the Emergency Response section was notified of this incident. A voicemail message requesting a return phone call was left for Mr. Seibert (614-620-3662) at 1:22 PM. At 5:00 PM I spoke to Mr. Seibert via phone conversation regarding this incident. Mr. Seibert was advised that clean-up would need to begin immediately. It was made clear that clean-up would consist of excavation of contaminated soil outside and within the dike. Excavated soil would need to be hauled to a landfill or land farmed where it could be remediated. Clean-up will be witnessed by an inspector. Mr. Seibert failed to notify ODNR in a timely manner of this incident in accordance with OAC 1501:9-8-02 Incident Notifications which took effect August 9, 2016.
