0361569062 2012-07-02 Annular Disposal Violations Noted An inspection of this well was conducted, in response to a complaint alleging the disruption of water well. During the inspection, it was noted that the well was equipped and showed signs of production. The tank battery location consisted of (3) 100 bbl tanks. A GPS reading was obtained for the well and tank. While facing East, the well is routed to the the tank on right hand. This tank contained a leak at the heater tube location on bottom of tank consisting of 1 drip per second. Fluid approx 3 in height. High Range Quantabs placed in the fluid showed maximum units of 10 were exceeded. 8.8 units equals 6562 ppm (mg/L)Cl-. The second tank was not connected to the first, but contained an intermittent leak at heater. Old oil was observed near this location. Fluid level approx 1. A line from bottom of this tank transitioned into plastic and was routed back to the well location and found cut with old oil stained ground. The third tank did not appear to be connected at this time. A fourth 100 bbl tank with bottom valve open (tank empty) was observed outside the dike not connected and contained vegetation around area. Brine impacted soils were observed inside the dike contained deer tracks. This soil inside the dike was field tested with High Range Quantabs and showed 8.4 units which equals 5149 ppm (mg/L) Cl-. Identification was not observed at the well or tank(s). The well owner shall repair or replace leaking tanks; Remove all contaminated material from the location. 1206191589 2013-05-14 Production Wells Violations Noted This inspection was conducted as a NOV follow-up. The well owner, Mr. Bill Lavender Jr., is not currently registered. The inspection revealed that the well owner has not posted identification at the well head or tank battery.
1453293830 2016-01-15 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. The inspection revealed previous deficiencies have not been resolved. Proper identification has not been posted, the well owner has not registered well with the Division of Oil and Gas Resources Management (DOGRM), and brine-laden soil has not been removed from inside the dike. According to the Licking County tax parcel map the previous landowner/well owner Mr. Bill Lavender Jr. sold the property/well to a Mr. Paul Williams. As a well owner Mr. Williams must be registered with the DOGRM per Ohio Revised Code 1509-07. A Change of Owner Form 7 will need to be filled out and returned to the DOGRM Cental Office. Mr. Williams will have 60 days to bring well into compliance. Failure to comply may result in enforcement action. Time of inspection: 9:30 AM. Weather: 42*F, Overcast
1522677596 2018-04-02 Production Wells No Violations My inspection revealed that the well has not been brought into compliance. Mr. Brent Bear issued a compliance notice on 7/5/2012 for failure to legibly identify well and well operation causing pollution and contamination. The contaminated material has been removed from the tank battery area. I did not observe identification at the well or tank battery. It appears that the old tanks have been replaced with new tanks. The tank battery now consists of (2) 100 barrel tanks inside an earthen dike. The well has not been registered to the current owner. I contacted Mr. Paul Williams by phone on 5/3/18. I advised Mr. Williams that the well needs to be registered. Mr. Williams said he has been working on it but has not been able to contact the previous registered owner. Time of inspection: 9:15 AM.
