0183434221 2004-09-07 Production Wells Violations Noted Complaint was called in to OEPA on 09/05/04 from Pam Gerhard and was passed on to ODNR radio room, and then to Jeff Fry and myself. Inspection revealed an area of two (2) feet by eight (8) feet that had been affected by approx. five (5) gallons of crude oil. The pumper Ron Fortney advised me that he had received the complaint on Sunday night and had shut the well in. He then advised me that there was a water truck and a backhoe in route to flush the line and dig up the leak. They will place a clamp on the line or if it is too bad replace the line. He advised me that they would remove the contaminated soil and replace it. I advised him that I would return on or about the 14th to check on there progress. At 10:54 A.M. this date Mr. Gerhard called me and I advised him of my findings, and the actions the owner is taking. He advised me that he was on his way out there and no one had called him to advise they were doing any work. I advised him that the owner was planning to make repairs to the line and/or replace the line, and remove the dirt and replace it with new soil.

Identification was posted at the well. The tank battery consists of (2) 100 barrel tanks inside an earthen dike. Identification was posted at the tank battery. No violations. 1484681976 2017-01-17 Production Wells No Violations I received a phone call from the Emergency Response Supervisor Scott King, about a leak at this well at 7:30am. I told him I could go to the well to do a field inspection on it to see what has happened. When I arrived on location Ethen Bremer a pumper for M&R Investments was on location. He was also on site the night before when the fire department was called out to the well. He showed me where the oil has come to the surface at. He has built a tiny dike so it did not travel any further. Ethan let me know a couple of guys were on there way with material to soak up the oil. Around 10am Dale Smith, an employee of M&R Investments and a helper of his arrived to start cleaning up the oil. They soaked up what was at the surface with some pads then put down a couple of absorbing booms just in case the rain washed more oil to the surface. He will be contacting O.U.P.S before he does any digging because there are lines in the area. He also stated he would like for it to dry a little since it is in the peoples front yard. I told him that would be fine. All of the contamination that was at the surface has been cleaned up and the well is shut in. The pumper will be checking back regularly to see if any more oil surfaces.
