1520861537 2018-03-12 Production Wells No Violations My inspection revealed that the well was equipped with a pumping unit and motor. The tank battery consists of (1) 100 barrel tank. Identification was posted at the tank. I spoke with the well owner Mr. James Carpenter during the inspection. Mr. Carpenter lives at the residence that the well is located at. Mr. Carpenter is planning on plugging the well this year. I provided Mr. Carpenter with a plugging application. Time of inspection: 1:20 PM.

1887677397 2019-04-12 Production Wells No Violations I conducted a status check on this well today. I spoke with Mr. Carpenter at his house regarding this well. Mr. Carpenter said the well is not in production. The well was equipped with a pumping unit and motor. The well was hooked up to a 100 barrel tank. The last year that the well had production reported on it was 2009. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 3:35 PM.
1938086783 2018-05-09 Production Wells No Violations My inspection revealed that the well was equipped with a pumping unit and electric motor. A flow line was connected to a 100 barrel tank. Identification was posted at the tank. The well was hooked up and capable of production. The well owner Mr. James Carpenter was home during the inspection. Mr. Carpenter lives on the property that the well is located on. I advised Mr. Carpenter that the Nichols #1 well is still registered to him. Mr. Carpenter said the well never belonged to him but that he is willing to do whatever it takes to get it out of his name. Time of inspection: 12:40 PM. Weather: 83 degrees F, Sunny
