Vegetation has been established at the tank battery location. At 3:06 PM, the well owner, Mr. Dwayne Dugan, was notified of inspection results by telephone.

1492381824 2011-05-23 Production Wells No Violations FOLLOW UP TO NOV # -673695820 The Division received a complaint alleging delinquent Final Restoration and pollution and contamination. An inspection revealed that a 50 bbl tank inside an earthen dike, which measured 40.5 x 14 x 3 (backside) has reached maximum fluid capacity. The top of this tank contained a hole in the lid allowing rainfall to enter the tank and overflow onto the ground surface. Pollution and contamination inside the dike at the tank location consisted of oil and brine which measured 22 x 14. The front section this dike did not contain a wall therefore, runoff from this location extended 155 (length) x 2 -8 (width) along the driveway. High Range Quantabs placed at 10, 62, and 118 intervals showed 5.2 units, 1365 ppm (mg/L) Cl-; 4.8 units, 1157 ppm (mg/L)Cl-; and 2.8 units 441 ppm (mg/L) Cl- respectively. Note: A fiberglass tank would have also remained inside the dike, but have been removed (pieces of this tank remain inside the dike). All pollution and contamination shall be removed from this location. The Final Restoration does not meet ORC 1509.072. The well owner shall remove all production and storage structures, oil and saltwater and fill remaining excavations. During the inspection, it was noted that a 50 bbl tank and concrete pad remained inside an earthen dike. The earthen dike shall be filled in and vegetation established. Above ground plastic, (3) electric poles with line, steel cable were also observed on this lease. No observed vegetation at the well site. The well owner shall establish vegetative cover to prevent substantial erosion. At conclusion of the inspection, the well owner was notified by telephone of the inspection results and required cleanup.
