0342798474 2015-03-23 Production Wells No Violations This inspection was conducted as a compliance follow-up. I arrived to the well location at 9:00 AM. The inspection revealed that oil and water has been removed. Mr. Bauer began excavating soil into a dump truck. One truck load of soil was removed. The soil was hauled to an EPA approved landfill. Mr. Bauer agreed to have new soil brought in at a later date. The well location will be in compliance when vegetation is established. Weather: 30*F, Overcast.

I was requested by the UIC section manager Kenny Brown to check if the well is being used for annular disposal. My inspection revealed that the well is not being used for annular disposal. During the inspection Justin Kelly was at the well when I arrived. Justin was there to pump the well. 1047341470 2015-02-10 Production Wells No Violations This inspection was conducted as a compliance follow-up for no identification. The inspection revealed pollution and contamination near the wellhead. There were (2) 300 gallon plastic tanks at the wellhead. One tank had 1 plastic flow line going into it. Crude oil has leaked either from one of the tanks or the stuffing box and was observed around the well. It appears work has been done with an excavator to clean up the oil. The well was pumping during inspection. It also appears that oil has run over the side of the 100 barrel tank at the tank battery. The tank is inside an earthen dike. There was no oil inside the dike. Identification has not been posted at the well or tank battery. A phone call was placed to well owner Joe Bauer. Mr. Bauer was informed of the inspection and was advised to do the following: 1) Clean up oil as soon as possible, 2) Remove tanks at the wellhead, and 3) Post proper identification. 1155176115 2015-03-22 Production Wells Violations Noted I arrived to the well location at approximately 9:30 AM. The inspection revealed clean-up efforts have begun. Joe Bauer was soaking up oil with pads. A sump pump was used to pump out water and oil into 300 gallon plastic containers. Water was being circulated from the containers through a hose to wash oil down to the sump pump. This continued for the remainder of the day. Mr. Bauer had his excavator delivered during the inspection. The excavator would be used to haul out soil after the oil and water was pumped out. Photos were taken for documentation purposes.

1409057160 2014-08-14 Production Wells Violations Noted This inspection was conducted in regards to a phone call received from the Ohio EPA @ 10:41 AM concerning an oil spill due to lightning hitting a 100 barrel storage tank. Upon arrival to the location at approximately 1:00 PM OEPA OCS Mike Dalton was conducting remediation efforts. Jim Augustyn with the U.S. EPA was also on site. The inspection revealed that the spill occurred on 8/12/2014 and oil was found running through a grassy ditch the next day by a landowner. The oil had traveled to the North a distance of approximately 1,000 feet from the tank battery. No streams or bodies of water were impacted from this spill. A dam was constructed @ N 39.92434 W -82.23917 with an excavator operated by a contactor for the OEPA. Freshwater was being hauled in to wash oil down the ditch to the dam where the oil would then be sucked off the top with a vacuum truck. The well owner Mr. Joe Bauer arrived during inspection and was complying with the OEPA. Remediation efforts will be ongoing and will be coordinated by Mike Dalton. The tank battery consists of (1) 100 barrel tank and it was revealed that the tank did not have a dike. The well owner will be advised to build an earthen dike capable of holding 110% of the capacity of the tank battery when the well is put back into production.

The inspection revealed that the well was pumping into a 300 gallon tank next to the well without a dike. Identification was not posted. An excavator on location had done some work on the original dike. The plastic tanks have been removed. Remediation was complete as the siphon dams were closed and straw was spread. 1608496644 2015-03-22 Production Wells No Violations At approximately 11:50 PM on 3/21/2015 I received a phone call from Ohio EPA OSC Aaron Wolfe regarding a leaking well. I arrived to the well location at 12:30 AM on 3/22/2015. The well owner Mr. Joe Bauer was at the well accompanied by one of his workers. The inspection revealed that the stuffing box had leaked approximately 2-3 barrels of oil and traveled approximately 50 feet from the wellhead. Mr. Bauer was putting absorbent pads on the oil. The oil was contained and did not impact any bodies of water. OEPA OSC Aaron Wolfe arrived to the well at 2:00 AM. A remediation plan was discussed and it was decided that clean up would begin after sunrise. Weather: 40*F, Clear
