0039232494 2009-02-26 Salt Water Injection Well No Violations FOLLOW UP TO NOV # 1235672988 Jeff Fry and I inspected the well location. There was salt water brine leaking at the well head. The brine was leaking from the tubing and was spraying out from the well three feet and landing on the ground. The injection pressure was at 800 psi. We took video footage and photographs. Also, a Quantab test was used to test the brine water. The high range Quantab went off scale showing that the water was greater than 6000 parts per million chloride. At 10:02 am we telephoned the company to inform them and the first company man was on location within ten minutes to shut the well down. We met with Scott Sterling on location and another man named Jim. They placed buckets by the well head to catch the leaking brine. They informed us that the well would remain shut down until the tubing could be pulled and repaired.

Checked log on brine hauler truck: Wilsons Water Hauling UIC 309, license plate: PAQ 7297 and also King Petrolium truck UIC 187. Checked both trucks and brine hauler registrations were okay. The top vault was almost full capacity. We mentioned to Jim King to have that pumped out. The dyke had some standing water and they had a siphon hose to drain the water. We used a quantab and found that the water had 100 ppm Chloride. The barrell counter in the pump house was broken. We also mentioned to Jim King to have the well head identification tag re-marked.

0216235386 2009-03-09 Salt Water Injection Well No Violations Jeff Fry and I witnessed rig operations today. The tubing was run back in the well to a depth of 6192 feet. This was done in order to push the packer below the perfs just in case the overshot tool was unable to grab the packer. The tubing was then pulled out of the well. The overshot tool did grab the packer and 10 foot piece of sheared off tubing. After the packer was recovered, they began sand-pumping on the well to clean out the bottom. The tally on the tubing showed that the records were wrong and the packer was actually set at 5710 feet.

The well was loaded with about 10 gallons of washer fluid, the air was bled off, and pressured up to 302 psi using an operators digital gauge and a DOGRM 1000 psi gauge. The pressure was monitored for 15 minutes with zero pressure loss. The well passed the test and has no integrity issues.

1235672988 2009-02-26 Salt Water Injection Well Violations Noted Jeff Fry and I inspected the well location. There was salt water brine leaking at the well head. The brine was leaking from the tubing and was spraying out from the well three feet and landing on the ground. The injection pressure was at 800 psi. We took video footage and photographs. Also, a Quantab test was used to test the brine water. The high range Quantab went off scale showing that the water was greater than 6000 parts per million chloride. At 10:02 am we telephoned the company to inform them and the first company man was on location within ten minutes to shut the well down. We met with Scott Sterling on location and another man named Jim. They placed buckets by the well head to catch the leaking brine. They informed us that the well would remain shut down until the tubing could be pulled and repaired.

Producers Services would mix inhibitor with water and circulate to surface. After setting the packer, multiple pressure tests were performed, pulled more tension, checked and fixed noticeable leaks fixed at surface, but would not pass required test. The average psi loss per minute was 100- 120 psi. A gauge placed on the injection side of well showed no pressure equilization. A valve from annulus between the 4.5 and 8 casing also showed no signs of pressure or vacuum at surface. The packer may need to be set higher. Additional testing will be conducted next week after allowing well to set over weekend.

1250785989 2009-08-13 Salt Water Injection Well No Violations I witnessed the construction of the upper tank battery dike today. Jeff Watson, Dave Hackenberry, and Larry Marks were all on location. The tank battery dike exceeds that of what was recommended by the Division. The recommended size was 125 feet by 27 feet by 2.5 feet deep. The finished dike size ended up being approximately 140 feet long, 30 feet wide, and about 3.5 feet deep. There were four inches of sand placed below and above a 30 mil one piece liner. An additional liner was placed on this top layer of sand and another four inches of sand and screenings were placed upon this second liner. Four new 300 barrel tanks built by Roads Tanks are being placed inside the dike with new scaffolding. A-51 tar was painted on the bottom of the tanks. SUR-11 plastic piping will be fused together for the line leading to the tanks from the unloading pad. Many photographs were taken of the construction process. Also, samples of the two liners, sand, and screenings were taken. The injection well was also tested. There was 310 psi on the annulus and 840 psi on the injection. The well was running at the time of the testing.

1322853827 2011-12-02 Salt Water Injection Well Violations Noted At 6:28 am, Mr. Jim Priest with Ohio Oil Gathering called and left a message stating that the tubing moved up hole and broke off. Fluid was released and cleanup has been started. Returned call and Mr. Priest states that problem was found this morning at 2:30AM during his inspection. An inspection revealed a 20.9 section of tubing broke off (not at collar) from remainder of string and was observed on the ground. A 7 7.5 section was cut off and now only 2 10.5 section of tubing extends above wellhead. A gauge placed on the injection side of well showed 20 psi and 10 psi on the annulus. A review of pressure chart at the well location, showed an abrupt decline of both injection and annulus pressures at time of incident. The injection pressure dropped off from 1000 psi and annulus psi of just under 600. Maximum injection pressure was never exceeded. Crew and equipment at location and have begun cleanup. The immediate area surrounding this well has been excavated and placed in a pit, which contained a liner. The impact extended from the well location to the East approximately 200 (length) into a low lying field with pockets of standing water near Priest Run. Note: Rainfall events this week have saturated the ground. A dam was built, in the field, to prevent entry into stream from a ditch line. A water truck removed fluid from an area with pooled water away from this stream approx >50 containing Cl in standing water. Quantity released is unknown at this time. No standing crude oil was observed. A truck load of lime in bulk arrived at location and began applying to area in field at 11:30 am. The tank was approx 3/4 full and a fitting located on the bottom of this tank contained a leak approx 1 drip per second. Pollution and contamination consisting of mostly oil remained on concrete and in the general area surrounding this tank. The storage tank location on top of hill (utilized by most local brine haulers) would also require general housekeeping around the concrete pad location. The required identification at the well and storage tank location(s) have now faded and would need to be completed. The area which had been impacted from the spill had been seeded, mulched and fertilized, but contained minimal vegetative growth (few rainfall events). Notified Mr. Jim Priest by cell phone at 2:30 PM. Mr. Priest agreed to resolve these deficiencies. At 4:55, I received a call from Mr. Priest stating that employee at location repairing the half steel tank and filling out the identification stickers.

Arrived at location within 15 minutes of receiving call and observed well shut down and no leakage at this time. A company representative was on location and cleanup crew enroute. The source of leak were the threads on 2 steel injection line at connection into the high pressure hose and located at the well. Note: Rainfall event today. High Range Quantabs placed in fluid (puddles) to determine area of impact exceeded maximum of 10 units. While facing North, the impacted area measured approx 15 radius facing to Northeast and Northwest direction. An embankment to the West approx 25 away allowed fluid to extend downhill into a field approx 125. The company was given two options for cleanup: dig/haul or bioremediation of impacted area. The company agreed to remove pooled fluid and opted for bioremediation of impacted areas with the application of lime, horse manure, seed. mulch and tillage in drier weather. Although not required for this facility, the construction of an earthen dike around the well was recommended to prevent future offsite impact.

The tubing had 200 psi. The pump was not running. The pressure gauge read 0 psi on the annulus. The gauge was removed and the annulus was on vacuum. OOGC was contacted and arrived to the well during inspection. UIC Inspector Tom Benko was also contacted and arrived during inspection. An employee for OOGC filled the backside with approximately 5 gallons of washer fluid until it was full. A mini integrity test was requested. OOGC agreed to conduct the test and would need a couple days to get their tools and equipment ready.
1390851050 2011-12-13 Salt Water Injection Well No Violations The rebuilt Haliburton R4 Packer on 2 7/8 tubing was unset/released to allow Producers Service Corp to circulate packer inhibitor. The packer was again reset at 5532 with approximately 20,000 lbs of tension. The annulus between tubing and 4.5 casing (longstring) was then pressured up to 1550 psi. The well passed the required test with no pressure loss in 15 minutes. Annulus pressure was blown down to leave a positive pressure of approximately 800 psi. 1,000 gallons of acid (15%) was then pumped down the tubing to conclude job. A verbal notice to the well owner had been granted on 12/12/11 to allow for setting packer at 5532. Today, the well owner will be submitting a variance request, in writing, to Tom Tomastik.

The cause of incident was due to tubing failure resulting in approximately 28 of 2 3/8 coming up hole and breaking off at the surface. Only 5 of tubing would be observed inside the well and remainder would be fished at depth of 478 with tool. Approximately 16 joints were added to the tool to allow retrieval of all tubing. The packer was also retrieved but found to be missing rubbers. The well owner will install new 2 7/8 tubing and rebuilt packer. On 12/3/2011, the same day of incident, cleanup was performed on the impacted area. 28 ton of lime was applied along with seed consisting of 1 bushel of cereal rye, 1 bushel wheat and 50 lb of Kentucky 31. Additional remediation will also be conducted in the Spring to ensure vegetative growth.
