A 100 barrel tank has been added to the tank battery. It appears that this tank is being used for brine for API permit numbers 4449, 4620, 4300, and 4925. Sawdust material has been applied to the oil contamination from the separator but the contamination has not been removed. I did not observe a leak from the heat tube on the 200 barrel tank. I did not observe identification at the well or tank battery. Prior to my inspection I drove to Mr. James Hindels house and knocked on his door but no one answered. I attempted to contact Mr. Hindel by phone but did not reach him. After the inspection I drove to Mr. Carpenters residence and spoke with him. 0917275073 2017-12-18 Production Wells Violations Noted I conducted this inspection in regards to a complaint regarding a gas leak, oil residue, and vegetation distress. The well was equipped with a pumping unit and motor. My inspection revealed that the area within 10 feet of the well was covered with saw dust. It appeared that the stuffing box or flow line has leaked recently and caused oil to spill onto the ground. Saw dust was also applied to a path from the wellhead approximately 30 feet in length east of the well. That appeared to be the leading edge of the spill. The well was not leaking gas or oil during the inspection. Identification was not observed at the well or tank battery. The tank battery consists of (2) approximately 200 barrel tanks and a separator. I observed a 100 barrel tank at the tank battery that is not associated with this well. The 100 barrel tank was identified as Dalton Production and is associated with API permit numbers 4300, 4449, 4620, and 4925. I left a voicemail message with the phone number listed for Inland Petroleum regarding this inspection. The well owner shall remove oil contaminated soil within 5 days and post identification within 10 days. Time of inspection: 12:40 PM. Weather: 40*F, Overcast

1299238619 2018-05-03 Production Wells No Violations I came back today to field test the soil for chlorides on the area outside of the dike. I took a small scoop of soil and placed it into a cup. I poured some bottled water into the cup and gently shook it up. I placed a high range Quantab titrator into the cup to test for chlorides. The test resulted with a reading of 5.2 Quantab units which equals 1275 ppm for chlorides. Time of inspection: 8:50 AM. Weather: 76 degrees F, Partly cloudy

The area within five feet of the wellhead was saturated with oil. I did not observe identification at the well or tank battery. The well has not been registered to the correct owner. 1525357698 2018-05-02 Production Wells No Violations I conducted a compliance notice follow-up on this well today. A complaint was received on 12/18/2017 regarding a chronic gas leak with some associated oil residue and vegetation distress. I issued a compliance notice on 12/19/2017 to Inland Petroleum as the well was in violation of ORC 1509.22 and OAC 1501:9-9-05 (A)10. My follow-up inspection revealed that no clean-up has been done around the well and identification has not been posted. During this inspection I found new pollution and contamination issues. Approximately 40 feet from the tank battery in the direction of the well I observed a small pool of oil approximately 2 feet by 1 foot in area on the ground. The separator at the tank battery is not hooked up but I observed fresh oil leaking from under the separator. A 15 foot by 10 area around the separator is contaminated with oil. A brine leak was observed coming from the heater tube on the 200 barrel tank closest to Montgomery Rd. I placed high range Quantab titrator in the heater tube to test for chlorides. The test resulted in a reading of 10 Quantab units which equals >6215 ppm chlorides. The bottom of this tank is in extremely poor condition and I observed salt crystals on the tank and ground near the tank. The dike for the tank battery does not appear to be capable of adequately containing the capacity of the tanks. The 100 barrel tank observed at the tank battery is associated with API# permit numbers 4925, 4449, 4620, and 4300. The identification on this tank is labeled as Dalton Lee. It appears that the leaking brine tank is being shared by Dalton Lee and Inland Petroleum. I took several photos of the well and tank battery. I placed a phone call to Mr. James Hindel at 2:20 PM but did not get an answer. I was not able to leave a voicemail message. The well is still registered to Inland Petroleum. On 3/12/2018 I visited with Mr. James Carpenter (Inland Petroleum) and discussed the compliance issues with the Nichols #1 well. Mr. Carpenter said that the w

2099679615 2017-12-29 Production Wells No Violations I conducted a follow-up inspection on this well today. When I arrived to the well Mr. James Hindel was present. Mr. Hindel told me that he is the well owner. I advised him that he needs to have the well registered into his name. I told Mr. Hindel that a registration packet would be sent to him. I asked Mr. Hindel about the leak and he said that the flow line busted causing the oil to leak onto the ground. I told him that when the weather improves that the area impacted from the leak would need to be remediated. The ground was frozen during the inspection and the temperature at the time of inspection was approximately 16 degrees Farenheit. Time of inspection: 8:40 AM.
