The valve was open on swage and gasing from inside the 4.5 casing. The valve was also open on annulus between the 4.5 and 7 casing(s). Fluid was observed between 7 and 9.63 (conductor) annulus with no discharge or bubbling. The liners for (2) drilling pits have fallen and also contained rips therefore allowing fluid behind. High Range Quantabs were placed in the fluid and showed 7.8 units, which equals 3755 ppm (mg/L) Cl-. (3) 100 bbl frac tanks were observed at the location. The 7/10/2012 frac job was not witnessed, due to insufficient notification. The required 24 hour notification was not met and would receive call from Mr. Jack Greene, a representative with the Oxford Oil Company, while frac job was just starting. Mr. Greene confirmed that he forgot to call sooner and agreed to meet the required notification for future jobs. Prior to hearing this alleged incident, I spoke with Mr. Greene, earlier this day at 9:39 AM by telephone in regards to the new Well Construction Rules (OAC) effective 8/1/2012. There was no mention of this incident. I called Mr. Greene at 4:52 PM, and confirmed casing failure on this frac job. Mr. Greene states they were 1/3 through frac job on sand, and with valve open on the annulus between 4.5 casing and 8 casing, circulation (not a full stream) was observed. They had good breakdown pressure and were on flush when this occurred. The 4.5 casing had been tested and they never lost pressure. The crew shutdown (did not complete frac) and would later bring in Cemenco to run a wireline, which showed possible hole or thread problem at 820. Mr. Greene states that they have scheduled Mr. Mike Wheelers service rig to repair casing (i.e. Pull tension, backoff casing below 820 and replace section of casing). Mr. Greene was again verbally instructed to provide sufficient notification and if problems were enountered on frac jobs. |
