0308072902 2017-06-29 Orphan No Violations This inspection was conducted as a status check. I accompanied Jeff Fry for this inspection. A new park is currently being built on the property. The well and the property were assessed due to the ongoing construction. Refer to Jeff Frys inspection report for details. Time of inspection: 9:00 AM.
0970071772 2011-11-23 Orphan No Violations An orphan well complaint was received by Central Office and referred via email. Attempts to contact the out of state complainant on 8/30/2011 were not successful with listed number. On 8/31/2011, the complainant was contacted by telephone. The complainant resides in PA but family owns property in Hanover, Ohio that contains an old well with above ground steel casing. She requested to assist with locating the well and will be returning to Ohio around Thanksgiving. An inspection conducted with the complainant on 11/23/11, revealed approx 8 above ground casing with fluid near the top of casing. Intermittent bubbling was observed within the fluid with no detection of gas with meter. Quantabs were placed in a sample of fluid and showed no Cl-. A stick was placed in the wellbore to gauge for any obstructions. Approx 3 down from the surface, a solid obstruction resembling cement was encountered. RBDMS shows well status as PA with no records or plug and abandon date. Approx 4 away from this well, a 7 casing was approx 1 above ground. There were no obstructions within this casing to approx 4. This wellbore is believed to have been the water well associated with the drilling of PN 61137. Access to the well would require crossing an intermittent stream/ditch. A GPS reading was obtained for the wellbore.

1429296236 2015-04-17 Orphan No Violations This well is in the orphan well program. This Inspection was conducted with DOGRM Licking County Inspector Brandon Mitchell to update files per the request of the Orphan Well Program Manager. It appears that this is an old standard rig well with 8.625 OD casing (8 ID) sticking up 24 above ground level (GL). Fluid level was at 10 from the top of the casing and natural gas is intermittently bubbling up through this fluid. No oil sheen or petroleum odors were detected. The Lower Explosive Limit (LEL) reading taken from inside of the 8.625 diameter casing registered 2 LEL when the gas would bubble and then it would drop back to 0 LEL. I tagged an obstruction inside this casing at a depth of 38. The GPS coordinates of this well are 40.07554, -82.27296. There is an associated water well located approximately 50 to the west of 61137. This water well is equipped with 7 diameter casing (6.5 ID). This casing is 20 inches above GL. Fluid level inside this casing is 36 from the well top. I tagged an obstruction inside this casing at a depth of 8 . Fluid level is static with no petroleum odor or surface sheen. The GPS coordinates of this water well is: 40.07554, -82.27298. There is a T-shaped concrete slab, which is approximately 10 long and 11 wide, which probably housed the motor for the old standard rig. The GPS coordinates of this concrete slab are 40.07559, -82.27318. Entrance access (GPS: 40.07864, -82.27596) is the lease road for permit # 5034, on the Mears property. There is an intermittent stream that will have to be crossed after entering the crop field. Photos (12) were taken.
1533665944 2018-07-26 Orphan No Violations Reviewed site with safety team, ODNR Oil and Gas Trish Cartmell, Jason Blocker, Jason McClarren, and Dan Terrell. We drove to the location and entrance. Well is in standing corn, we did not walk to the well.
1544837023 2017-06-29 Orphan No Violations This well is in the Orphan well Program. Site visit update with ODNR DOGRM Licking County Inspector Brandon Mitchell. This well site has two steel casings which was an old standard rig drilled well and water well. The 8.625 outside diameter casing continues to intermittently leak gas bubbles through fluid on the inside of the casing. This would have been the abandoned production well. The area around the casings that used to be weeds and brush has been cleaned off and is now worked up for an agricultural field. The area or field across the drainage ditch opposite of the well (to the north), is being developed for a Community Public Park by the Wyatt Adkins Heart Organization. There is a water well rig drilling a water well on this property. This property used to be in the Mears Family Trust and is now owned by Carl Hains. We met Carl Hains on location and confirmed the right of entry and informed him of updates on the well (s). I took 12 additional photos and a movie of the leaking well. I will update the Risk Evaluation Matrix.
1554584747 2012-09-21 Orphan No Violations A follow-up inspection revealed a more frequent occurrence of gas bubbling inside the 8 casing. A gas odor was not only smelled, but also detected with a meter. The following readings were observed: O2= 21.0; LEL= 4-6; CH4-IR= 0.15. Also noted was the intermittent buildup of gas rumbling could be heard from this well. The 8 casing was not equipped with a wellhead and measured 2 above ground level with fluid level at 10 from the top of casing. There was no oil or brine observed within this fluid. High Range Quantabs placed in a sample of fluid showed 0.2 units and compliance with US EPA recommended aesthetic value of 250 ppm (mg/L) Cl-. No overhead obstructions exist around the well. The well is situated in a un-planted field. A solid obstruction resembling cement was verified 3 down hole. RBDMS well status indicated PA with no plug report or well history and the Ohio Fuel Gas map showed a Dry Hole drilled to the Clinton formation. This referral will consist of reopening to the Clinton to plug and abandon the oil and gas well and removing the 11x11 concrete pad from the property. The previously noted water well with 7 casing shall be cemented from total depth to the surface. Well Access: Utilize PN 5034 lease rd entrance also located on the Mears prop. Approx 1500 of total rd required between crop fields and ditch/intermittent stream crossing. A GPS reading was obtained for the access road entrance.
