0054996472 2012-07-10 Production Wells Violations Noted The Division had received a complaint alleging two wells owned by Everett Oil Company were idle. The landowner further alleged water broke-in 20-25 years ago and well has not pumped for last 10 years. On 2/21/2012, an inspection confirmed that this well was idle. An appointed representative for this company, Mr. Mark Hardesty, was verbally instructed to plug or produce by 6/1/2012 prior to the issuance of this Compliance Notice. On 7/10/2012, a follow-up inspection revealed non-compliance and well remains incapable of production. The valves were closed and the production line, as previously noted had been cut in several places. The tank battery locations consisted of (2) 100 bbl tanks located in the woods, across from the (3) 100 bbl tanks and surrounded by thick brush and vegetation. Mr. Hardesty had recently confirmed that they made an attempt to produce the well, but water has broke-in on the casing. At time of inspection, this well was currently listed as Unknown Owner with no reported history of reported production or frac. The following will be required: (1) Provide documentation to Ms. Erica Freeman with the Division of Oil & Gas Resources Management that said well is owned by the Everett Oil Company. (2) File for a Plug and Abandon Permit. (3) Plug and Abandon said well within 30 days.
0423223516 2012-02-21 Production Wells No Violations FOLLOW UP TO NOV # -1496233578 The Division received a landowner complaint alleging idle well owned by the Edward Everett Company. An inspection revealed that the well was idle. The well was equipped with a pumping unit and engine however, the valves were closed and the production line from the pumping tee side of well was cut. The landowner alleges that water broke-in 20-25 years ago therefore, no gas produced, and has not pumped for approx 10 years. Verified the well location with a GPS. At time of inspection, this well was currently listed as Unknown Owner with no reported production. The identiification at the well was not legible and complete with reference to Flenner well. As previously noted, in non-related well reports, the well owner is deceased. Notified Mr. Mark Hardesty, an appointed representative of the estate, of inspection results and agreed to plug or produce well by June 1, 2012. Central Office will be notified of Unknown Owner status.

1694446025 2017-06-07 Final Restoration Violations Noted This inspection was conducted as a final restoration status check. The inspection revealed that the final restoration requirements have not been met. During the inspection 2 inch black plastic flow line and 1 inch orange plastic flow line was osberved. The well location has been graded and vegetation established. The tanks and pumping units have been removed. Photos were taken of the plastic flow line. The final restoration will be complete when the flow lines are removed. Time of inspection: 1:40 PM.
1798933227 2012-08-06 Production Wells No Violations A follow-up inspection was conducted, in order to obtain the assigned shipping number(s) for this well, which are typically displayed at the tank battery location and not regulated by the Division. The well had allegedly been owned by the Edward H. Everett Co. Further documentation would be required to prove this allegation therefore, the shipping numbers were obtained to research oil shipped and last known well owner. The shared tank battery consisted of (3) 100 bbl tanks for Permit No. 61204 and Permit No. 3202. The displayed shipping number on first 100 bbl tank showed 106609. This tank also contained an identification plate stating EH Everett as owner with lease name stating Russel-Flenner. The second 100 bbl closest to the lease road contained a shipping tank number of 106010. This tank also contained identification, but had been stenciled with paint to show Permit No. 3202 and Permit No. 2323 with lease name displayed as Russel & Flenner. Note: Permit No. 2323 was mistakenly displayed because, this well had been plugged and abandoned on 3/15/1966 and located on opposite side of road and approx 3000 away from Permit No, 61204. The Flenner well in question is Permit No. 61204. Prior to inspection, this case was discussed with Oil & Gas West Region Supervisor, Mr. Will Ziegler.
