0085711745 2012-04-09 Administrative Inspection No Violations At 8:54 AM, I called Mr. Bud Porterfield to discuss landowner complaint and status of well(s). Mr. Porterfield confirmed non-production for several years and has focused his attention on wells in Kentucky. He mentioned non-related wells and not on Stevens property, in which he let a guy have these wells and called him a year ago and nothing was done. Informed that wells still remain under his name and when asked if there would be an assignment he stated at Licking Co. Recorders office he stated that attorney took care of transfer and was not sure. Mr. Porterfield stated that he talked to 3-4 people about wells on Stevens property and will talk with his attorney today. Informed Mr. Porterfield of SB 165, a recent change to Ohio oil and gas law and asked for cooperation. Mr. Porterfield agreed to work with the Division to achieve compliance and will talk with attorney today. At 3:28 PM, I had a missed call from Mr. Porterfield. I returned his call and answered questions about well ownership transfer and provided Erica Freemans contact number at Central Office. Mr. Porterfield would like to sell all wells on Stevens property to company and will also work on transfer of wells on non-related property. The wells listed under Wallick Petroleum and Unknown Owner, on Stevens property, were also brought to his attention, in which he confirmed ownership. Central Office will be notified of ownership status.
0176872753 2018-02-12 Production Wells No Violations I conducted a compliance notice follow-up on this well today. My inspection revealed that the well has not been plugged or placed into production. It was not capable of production as there was no pumping unit on the well. I issued a compliance notice on 4/14/2016 to have the well plugged. The well was also in violation for failure to identify well and failure to report production. Mr. Bevard has posted identification and reported production since the compliance notice was issued. I placed a phone call to Mr. Bevard at 2:38 PM and spoke to him about bringing the well into compliance. Mr. Bevard said he needs to put his rig on the well to evaluate if it needs plugged or not. I took one photo of the well. A Form 7A will be sent to Mr. Bevard with instructions on how to register a Historic Owner well into his name. Time of inspection: 11:40 AM. Weather: 29*F, Sunny

It was not capable of production. Identification was not posted at the well. RBDMS research revealed no production reports for this well. The well was sold to H & A Drilling/Carl W. Bevard from Owner #2085 Nelson Bud Porterfield. It is not currently registered with the Division. H & A Drilling will need to complete a Form 7 and Form 7A to register well per Central Office. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509.12(B), and plug the well within 180 days. An inspection revealed an idle well with a pumping unit, no motor or electric. No house gas received from this well. The polish rod showed no signs of production and flowline from pumping tee was not connected to the tank battery. Note: Tank battery consisting of (3) 100 bbl tanks are located opposite side of road on SR 586. A GPS reading obtained for the well. No identification observed at well or tank battery. At time of inspection, this well was mistakenly listed as Unknown Owner., and not placed under Mr. Bud Porterfield. Mr. Porterfield confirmed well ownership and the landowner has also stated that he owns the well. M. Porterfield will need to complete Form 7A for well ownership per Central Office. Records show no reported production. On 4/9/2012, at 8:54 AM, Mr. Porterfield was verbally informed of complaint and status of well(s) and instructed to plug or produce. Furthermore, additional wells would need to be inspected to cover all wells on Stevens property. Mr. Porterfield stated that he sold wells 2-3 years ago on non-related property to a guy and called 1 year ago and nothing done; No assignment at Licking Co. Recorders Office; Spoke with 3-4 people about wells on Stevens property; and would talk with his attorney today.
