0655547509 2016-07-08 Production Wells No Violations Inspect well as a status check. The well was equipped with a pumping unit and motor. This well is an old standard rig well and has the original metal derrick on it. The tank battery consists of (1) 100 barrel tank and (1) 1600 gallon plastic brine tank. Identification was not observed at the well or tank. A permit number was assigned to the well as it did not appear to have one before. The well is owned by H & D Drilling. The company will be advised to register well under their bond. Time of inspection: 12:50 PM.
1075262782 2017-12-05 Production Wells No Violations I conducted a follow-up inspection on the tank battery location. The plastic tank and pieces were removed. The oil has been cleaned up. A phone call was placed to Mr. Harold Donaker. Mr. Donaker said he is planning bringing in a load of gravel and then setting a new tank. The clean up has been finished. Time of inspection: 2:45 PM. 1595823786 2017-12-01 Production Wells No Violations I inspected the tank battery location as a follow-up in regards to the plastic tank being hit by a car. The oil was not cleaned up as of the inspection. The busted tank and pieces of the tank was not removed. A phone call was placed to well owner Mr. Harold Donaker and he was advised to get it cleaned up. Mr. Donaker acknowledged it needed cleaned up and said he would work on it either today or over the weekend. Time of inspection: 12:40 PM.
1688329872 2017-11-29 Production Wells Violations Noted I conducted this inspection as a status check. My inspection revealed that a 1600 gallon plastic tank had been busted into several pieces and oil was released onto the ground around the tank. This tank was used as a brine storage tank. The amount of oil observed was approximately 20 gallons and did not impact any ditches/streams. I placed a phone call to the well owner Mr. Harold Donaker. Mr. Donaker said that a car drove through the tank and caused the release. I advised Mr. Donaker that the oil needs to be cleaned up and that the well needs to be registered to H & D Drilling. Time of inspection: 12:15 PM.
