1. Hole condition: no bubbling in fluid. I used my Sperian gas meter to check for natural gas - no gas detected. 2. I checked with fire department personnel to confirm status of gas readings in and around the well area and vent locations - all have been 0 readings. 3. The contractor is milling out the 5 5/8 diameter casing using a 6.75 diameter mill. Over drill casing at casing (GOOD SIGN). 4. Drilling slowly at 156 to stay on the 5 5/8diameter casing, cutting showing small bits of shale allot of metal. Another 10 to the swedge and drill time to drill thought 3-4 hours. 5. The object is to Drilling to approximately 190 with the 6.75 mill then trip out the mill and 2 7/8 drill stem. Then work on getting the 8 5/8 over drill casing and bit. Then ream surface hole to 11.75 to approximately 190 and run the 8 5/8 casing to approximately 168 and cement it. |
0480688597 | 2014-11-05 | Orphan | No Violations | This follow up inspection was conducted on 11/05/14 to view the excavating of the exit side of the driveway of the north parking lot of the school, approximately 50 feet to the west of Washington Avenue, in order to determine if an orphan well (34-093-2-1209) was located in this area. The Surveyor used the GPS and plat measurements shown in our database for this well.
1. A power saw was used to cut an area approximately 6 X 8 in the asphalt. 2. Once the asphalt was removed I used my Sperian gas meter to check for natural gas - none was detected. 3. As the excavating continued deeper I used my Sperian gas meter to monitor the excavated area. The readings are as follows: - 0% gas readings at depth approximately 1. - 0% gas reading at depth approximately 2. 4. At depth of approximately 3 the contractor used an air hammer to break up the ground and then used a tile probe to check around the base of the excavation another 10 of depth for casing - nothing found. I also usied my Sperian gas meter to check for gas - none detected. 7. At depth of approximately 4 the contractor uncovered an 8 diameter cast iron pipe which was running horizontal. This pipe had several repairs that had band clamps. This pipe showed no indication of being active. I used my Sperian gas meter to check for natural gas but none detected in or around this pipe. The second area of interest, located approximately 170 feet west of Washington Avenue and just along outside of the north wall of school gym was also excavated. 1. Excavated along outside of the north wall of the gym, down approximately 3 to the footer and found nothing. We were unable to use metal casing locater because of metal present in the footer. I also used my Sperian Gas meter to check for natural gas - none detected. |

The Contractor called to get water truck to take a load of fluid off half tank. * Phone call to Gene Chini, Orphan Well Program Manager, to ask if I could ream hole to 148 and then set the 8 5/8 casing. He agreed. * Advised Contractor to pull the drill string and get ready to pull the over drill and ream hole to set the 8 5/8 surface casing. * The Contractor pulled drill string and found the 8 5/8 over drill casing is loose. |

The drill rate is approximately 10 minutes per foot. 2. Drilled to approximately 130, the drill cuttings showed shale and small bits of wood. 3. The Contractor plans on removing a load of fluid from the half tank and re-filling with fresh water (because the intake screen keeps getting clogged with shale and wood fibers). 4. Had short meeting with school administrators, Jeff Hawks and Tom Tucker to discuss plugging status. Checked on Century Link and Time Warner Cable line removal progress. |

Checked hole for gas with my Sperian meter - none detected. 2. 3 joints (approximatley 64.3) of the 8 5/8 diameter over drill casing was removed from the well bore when the medium chain tongs broke. The contractor cut the casing in order to remove this casing. The tally for this casing is: 22.11, 21.10, 21.10, 21.90 and 5, for a total of 91.21. The crew brought out a set of larger chain tongs to replace th broken ones. 4. The crew pumped out the fluid in the well bore and was able to see the 5 5/8 diameter casing laying off too one side of the well bore. 5. Contractor rigged up and retrieved the following amount of 5 5/8 diameter casing: 19,19.2,20.70,and 17.11, for a total of 76.01. The last joint of retrieved casing had several quarter sized holed in it and the end showed signs of being twisted off. |

I proceed to checked this fluid with my Sperian gas meter - no gas detected. * After the 6th connection on the 8.625 diameter over drill casing, the circulation pump was started and it pressured up with no circulation at the surface. The drill stem could not be rotated. The contractor tried to pull out and was unable to go up or down. The Contractor shut down in order to try to determine the problem. |

Drilled unconsolidated material (shale, red brick, sandstone and wood) from 0 to 30. 3. At 30 the contractor could tell the drill bit started to ream hole to size. 4. From 30-60 the drill rate is 10 minutes per foot. 5. Cuttings show black (Ohio) shale with small flecks of white material (sandstone or cement). 6. At a depth of approximately 90, the driller stated that the drill was not turning the way it should and he began to trip the drill stem out in order to check condition of the bit. |

This follow up inspection was conducted on 11-06-14 to excavate to locate the gas well in the school gymnasium area. I reviewed the gas readings collected by the Lorain Fire Department and found a pattern of high readings around the location plotted by surveyors, along the north or left foul line of the basketball court. Once the concrete was cut, I observed that there was no re-bar in the concrete. I used my magnetic metal locater and surveyed this area and found a high magnetic reading approximately 2 away from where the concrete had been cut. I advised the contractor to extend the cut area and excavate this area. At a depth of approximately 4.5 we found a concrete slab which was determined to be a 4 x 4 square red brick vault which had been filled concrete. The contractor removed all of the concrete from the vault and at approximately 5 deep found a C-Clamp well head. Attached to the well head was a 2 PVC pipe which terminated outside the vault. I checked for natural gas with my Sperian gas meter and obtained a reading of 10% LEL. The PVC piping had no mechanical connection to the well head, it was just setting on the top of well head. I called Orphan Well Program Manager to get approval for a contractor to install a power vent I then contacted Moore Well Service who dispatched a crew that arrived on site in 2 hours. They installed a system to vent the natural gas from around the well casing to the outside of the building until arrangements could be made to access the building with heavy equipment. |

This follow up inspection was conducted on 01-12-2015 for emergency contract compliance. *The Ready Mix truck from Terminal Ready -mix Inc. arrived at 11:45 am with a load of 9 Sack Grout mix and was positioned over the well bore. The Contractor ran 1.5 diameter plastic pipe into the surface casing to approximately 149. This pipe will be used as a siphon string to remove any water in the hole while placing the grout. The grout was thinned with water to the proper consistency and was placed into the well bore. The extremely low temperature caused the siphon string to freeze up several times but we were able to thaw the line and set the grout properly. Approximately 2.5 cubic yards of grout was used to fill the well casing to surface. |
