0472666440 2014-12-17 Orphan No Violations This follow up inspection was conducted on 12-17-14 for emergency compliance.

1. Hole condition: no bubbling in fluid. I used my Sperian gas meter to check for natural gas - no gas detected. 2. I checked with fire department personnel to confirm status of gas readings in and around the well area and vent 

    locations - all have been 0 readings.

3. The contractor is milling out the 5 5/8 diameter casing using a 6.75 diameter mill. Over drill casing at 

    casing (GOOD SIGN).

4. Drilling slowly at 156 to stay on the 5 5/8diameter casing, cutting showing small bits of 

    shale allot of metal. Another 10 to the swedge and drill time to drill thought 3-4 hours.

5. The object is to Drilling to approximately 190 with the 6.75 mill then trip out the mill and 

     2 7/8 drill stem. Then work on getting the 8 5/8 over drill casing and bit. Then ream surface
      hole to 11.75 to approximately 190 and run the 8 5/8 casing to approximately 168 and 
      cement it.
0480688597 2014-11-05 Orphan No Violations This follow up inspection was conducted on 11/05/14 to view the excavating of the exit side of the driveway of the north parking lot of the school, approximately 50 feet to the west of Washington Avenue, in order to determine if an orphan well (34-093-2-1209) was located in this area. The Surveyor used the GPS and plat measurements shown in our database for this well.

1. A power saw was used to cut an area approximately 6 X 8 in the asphalt. 2. Once the asphalt was removed I used my Sperian gas meter to check for natural gas - none was detected. 3. As the excavating continued deeper I used my Sperian gas meter to monitor the excavated area. The readings are as follows: - 0% gas readings at depth approximately 1. - 0% gas reading at depth approximately 2. 4. At depth of approximately 3 the contractor used an air hammer to break up the ground and then used a tile probe to check around the base of the excavation another 10 of depth for casing - nothing found. I also usied my Sperian gas meter to check for gas - none detected. 7. At depth of approximately 4 the contractor uncovered an 8 diameter cast iron pipe which was running horizontal. This pipe had several repairs that had band clamps. This pipe showed no indication of being active. I used my Sperian gas meter to check for natural gas but none detected in or around this pipe. 

The second area of interest, located approximately 170 feet west of Washington Avenue and just along outside of the north wall of school gym was also excavated.

1. Excavated along outside of the north wall of the gym, down approximately 3 to the footer and found nothing. We were unable to use metal casing locater because of metal present in the footer. I also used my Sperian Gas meter to check for natural gas - none detected.

0506991526 2014-12-18 Orphan No Violations This follow up inspection was conducted on 12-18-14 for emergency compliance.

* No bubbling detected in well fluid. I used my Sperian gas meter to check for natural gas - no gas detected. Because 

    of the communication with the 8 5/8 annulus the bubbling present in cellar was checked for natural gas with my
    meter. I obtained a reading of 1% on the CH4 but nothing on LEL.

* I checked with fire department personnel to confirm status of their gas readings - all have been 0 natural gas readings. * The Contractor is milling at 144-146 on metal , wood, wheat straw and cement. Drilling rate is 10 minutes per foot. 

      There is circulation between well and annulus. Lost approximately 4 of fluid in the half tank (5 bbls).
  • At approximately 148 there is no circulation up inside the 8 5/8 over drill casing but circulation on outside of this
    casing is bringing up wood and fine shale cuttings.

* The half tank is getting filled with fine shale cuttings, wood, metal and cement. The Contractor called to get water truck to take a load of fluid off half tank. * Phone call to Gene Chini, Orphan Well Program Manager, to ask if I could ream hole to 148 and then set the 8 5/8 casing. He agreed. * Advised Contractor to pull the drill string and get ready to pull the over drill and ream hole to set the 8 5/8 surface casing. * The Contractor pulled drill string and found the 8 5/8 over drill casing is loose.

0528218000 2014-11-19 Orphan No Violations Moore Well Service mobilized their equipment to the site. A pre-construction meeting was held with representatives from the school (Jeff Hawks), Lorain Fire Department (Jeff Fenn), Lorain Police Department (Reubin Figuerca and Jason Orellan) and DOGRM (Norburt Lowder, Gene Chini, Dave Claus). 0587032529 2014-12-19 Orphan No Violations This follow up inspection was conducted on 12-19-14 for emergency compliance.
  • The Contractor advised me that there was no bubbling in the fluid inside of the casing. However, while they started to
    circulate on the 8 5/8 casing they had a small gas show which lasted for approximately 10-15 seconds.
  • The Contractor stated that at approximately 55-65 the formation itook approximately 20 bbls fresh water.
  • I used my Sperian gas meter to check for natural gas within the well cellar - no gas was detected.
  • I checked with fire department personnel to confirm status of their gas reading and all have been 0 readings.
  • The Contractor is working on removing the 8 5/8 over drill casing but having trouble unscrewing the casing. They had
   to welded tabs to each collar for a better grip for the wrenches.
0616234464 2015-01-05 Orphan No Violations This follow up inspection was conducted on 01-05-15 for emergency compliance.
  • Checked with fire department personnel to confirm status of gas reading in and around well area and vents within and around the impacted structure locations. All sampled areas have had 0 % natural gas readings.

* I visually checked the fluid in cellar and no bubbling was observed. I also checked the cellar with my Sperian gas meter and no natural gas was detected. * I checked the pressure on well using the contractors electronic gauge, which registered 1 pound pressure. * The Contractor removed the swage that had been installed on the well and I saw no bubbling in the fluid on top of the cement. I also used my Sperian gas meter to check for natural gas and none was detected.

Moore Well Service transported the 8.625 diameter casing, drill collars, mud pump, 2 3/8 diameter tubing and rig to location. All but the mud pump was placed inside the gym area.

This follow up inspection was conducted on 12-08-14 for emergency compliance.

1. The Contractor tripped in the 8 5/8 diameter over drill pipe to approximately 95 to start washing over the 5 5/8 diameter casing. They had to rotate to get over the casing in hole. The drill rate is approximately 10 minutes per foot. 2. Drilled to approximately 130, the drill cuttings showed shale and small bits of wood. 3. The Contractor plans on removing a load of fluid from the half tank and re-filling with fresh water (because the intake screen keeps getting clogged with shale and wood fibers). 4. Had short meeting with school administrators, Jeff Hawks and Tom Tucker to discuss plugging status. Checked on Century Link and Time Warner Cable line removal progress.

1254546840 2014-12-05 Orphan No Violations Follow up inspection was conducted on 12-05-14 for emergency compliance.

The site condition before and during drilling operation are as follows:

1. Hole condition - no bubbling in fluid. Checked hole for gas with my Sperian meter - none detected. 2. 3 joints (approximatley 64.3) of the 8 5/8 diameter over drill casing was removed from the well bore when the medium chain tongs broke. The contractor cut the casing in order to remove this casing. The tally for this casing is: 22.11, 21.10, 21.10, 21.90 and 5, for a total of 91.21. The crew brought out a set of larger chain tongs to replace th broken ones. 4. The crew pumped out the fluid in the well bore and was able to see the 5 5/8 diameter casing laying off too one side of the well bore. 5. Contractor rigged up and retrieved the following amount of 5 5/8 diameter casing: 19,19.2,20.70,and 17.11, for a total of 76.01. The last joint of retrieved casing had several quarter sized holed in it and the end showed signs of being twisted off.

The Contractor prepared the well head for milling and then shut down for the Holiday weekend.

This follow up inspection was conducted on 12-16-14 for emergency compliance.

* No bubbling visible in the fluid in the casing. Used gas meter to check for natural gas - no gas detected.

* Checked with fire department personnel to confirm status of gas reading in and around area and vent locations - all have 0 readings.

* Using a 6.75 diameter mill to mill the 5 5/8 diameter casing.

I AM LOST AFTER THIS!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! * Depth 116 to 122 problem casing only being partial mill out had to fold over the parts of the casing that were sticking up so they can be cut.

  • Contractor rigged up a grapple to retrieve the rest of tubing, after pulling four times tubing came free retrieved approximately 15.
1365468063 2014-12-09 Orphan No Violations

This follow up inspection was conducted on 12-09-14 for emergency contract compliance. Conditions at the site are as follows: 

              * Hole condition - no bubbling in fluid.  Checked for gas with my Sperian meter - none   
                 detected.

* Drilling at approximately 130 with a drill rate of 5 minutes per foot. * The drill samples are showing black shale chips and wood fibers (approximately 1 long). * The circulating fluid is showing a release of dissolved gas. I proceed to checked this fluid 

                  with my Sperian gas meter - no gas detected.

* After the 6th connection on the 8.625 diameter over drill casing, the circulation pump 

                 was started and it pressured up with no circulation at the surface.  The drill stem could  
                 not be rotated.  The contractor tried to pull out and was unable to go up or down.

           The Contractor shut down in order to try to determine the problem.
1369333126 2014-12-01 Orphan No Violations This follow up inspection was conducted on 12-01-14 for emergency contract compliance.

1. No bubbling noted in the well fluid. I used my Sperian gas meter to check for natural gas - no gas detected.

2. Using a 6.75 tapered mill, the rig crew started to mill the 5 5/8 diameter casing. After a short time it was noted that the cuttings contained slivers of metal which indicated that the mill was not grinding the casing up but causing the casing to bell outward. Milling ceased at a depth of 5 and it was decided to over drill the 5 5/8 casing using 8 5/8 casing with bit fabricated out of an 8 5/8 collar.

1417976987 2014-12-04 Orphan No Violations

This followup inspection was conducted on 12-04-14 for emergency contract compliance.

Contractor and crew on site and rigging up to over drill the 5 5/8 diameter casing. The site conditionsare as follows: 

  1. This morning the fluid in the cellar was bubbling vigorously.  Checked with my Sperian meter - 
       no readings.

2. Drilled unconsolidated material (shale, red brick, sandstone and wood) from 0 to 30. 

  3.  At 30 the contractor could tell the drill bit started to ream hole to size.

4. From 30-60 the drill rate is 10 minutes per foot. 5. Cuttings show black (Ohio) shale with small flecks of white material (sandstone or cement). 6. At a depth of approximately 90, the driller stated that the drill was not turning the way it should 

       and he began to trip the drill stem out in order to check condition of the bit.
1418219859 2014-12-03 Orphan No Violations This follow up inspection was conducted on 12-03-14 for emergency contract compliance.

1. The Contractor and crew rigged up to over drill the 5 5/8 diameter casing. 2. The fluid in the cellar had no evidence of bubbling and no natural gas was detected with my Sperian meter. 2. The 42 long joints of 8.625 diameter casing, to be used to over drill, were cut in half (21.9). This cut length includes the collars and threaded connectors, which will be welded on. 3. From 10-20 the drill rate is 7 minutes a foot. Samples show Black (Ohio) shale with small flecks of white material, which could be sandstone or cement.

Also met with Columbus DOGRM personnel onsite and explain plugging activities and equipment.

1419432722 2014-11-04 Orphan No Violations This inspection was conducted on 11-04-14 during the excavation of the two well locations plotted by the surveyor using information from the ODNR data base.

* The first location plots in the school driveway, approximately 50 to the west of Washington Avenue. * The second location plots approximately 170 west of Washington Avenue and adjacent to the north wall of school gym.

I waited on site for approximately one hour for the the excavator that was contracted by the School System to arrive. I called School Administrator Jeff Hawks who informed me that this work was rescheduled for Wednesday (11/5) morning.

1420814587 2015-01-13 Orphan No Violations This follow up inspection was conducted on 01-13-15 for emergency contract compliance.

* Checked the cellar of the plugged well for natural gas with my Sperian gas meter - none detected. 

    There was also no bubbling observed in the fluid contained in the cellar.

* The level of grout did not fall and will not need to be topped off.

1423228002 2014-11-06 Orphan No Violations

This follow up inspection was conducted on 11-06-14 to excavate to locate the gas well in the school gymnasium area.

I reviewed the gas readings collected by the Lorain Fire Department and found a pattern of high readings around the location plotted by surveyors, along the north or left foul line of the basketball court.

Once the concrete was cut, I observed that there was no re-bar in the concrete. I used my magnetic metal locater and surveyed this area and found a high magnetic reading approximately 2 away from where the concrete had been cut. I advised the contractor to extend the cut area and excavate this area. At a depth of approximately 4.5 we found a concrete slab which was determined to be a 4 x 4 square red brick vault which had been filled concrete. The contractor removed all of the concrete from the vault and at approximately 5 deep found a C-Clamp well head. Attached to the well head was a 2 PVC pipe which terminated outside the vault. I checked for natural gas with my Sperian gas meter and obtained a reading of 10% LEL. The PVC piping had no mechanical connection to the well head, it was just setting on the top of well head.

I called Orphan Well Program Manager to get approval for a contractor to install a power vent I then contacted Moore Well Service who dispatched a crew that arrived on site in 2 hours. They installed a system to vent the natural gas from around the well casing to the outside of the building until arrangements could be made to access the building with heavy equipment.

1428858673 2014-11-24 Orphan No Violations This follow-up inspection was conducted on 11/24/2014 for emergency contract compliance.

The 4.5 well control head was removed in order to attempt to pull the 2.375 diameter tubing using an internal mechanical spear. Approximately 30,000 # of pull was used by the rig, with no movement of the tubing.

The rig crew prepared overshot drill bit using 4.5 diameter casing. They will use this to drill out the cement between the 2.375 diameter tubing and the 5.625 diameter casing. Drilled to a depth of 10 and shut down.

1429831043 2014-11-25 Orphan No Violations This follow-up inspection was conducted on 11/25/2014 for emergency contract compliance.

The Contractor prepared the drilling equipment to start drilling out the cement between the 2375 diameter tubing and the 5.625 diameter casing. They constructed an over-shot drill using 4.5 diameter casing with teeth cut into one end. The collars were cut off and the joints were welded as drilling progressed. Tripped out the hole at 140 and attempted to pull the 2.375 tubing. They were able to unscrew the tubing and recovered approximately 92, which was full of cement.

1432035322 2014-12-15 Orphan No Violations This follow up inspection was conducted on 12-15-14 for contract compliance.
  • Contractor rigged up and tripped in an external grapple on tubing, to see if they can get more of the existing 2.375 diameter tubing out of the well. They tagged the tubing at 120 and retrieved another 15 of tubing.

* Tripped in a 6.75 diameter flat mill to approximately 130. Cutting samples showed bits of wood, rope fibers, metal and cement. Milled to 135 and then shut down for the day.

1432831202 2014-12-22 Orphan No Violations 
No on-site inspection was conducted due to a mandatory staff meeting in Columbus.

I received a phone call from Contractor (Moore) advising me of site condition on 12-22-14:

After setting surface casing on 12-20-14, the Contractor found bubbling inside the surface casing. I advised the Contractor to drill out the cement in this casing to approximately 146. Contractor will be shutting down until after the holidays.

1896940988 2014-12-20 Orphan No Violations .Follow up inspection was conducted on 12-20-14 for emergency compliance.


  • The well bore was then reamed with a 10.75 tri-cone bit to approximately 146.2
  • Contractor tripped in well bore with 8.6250 diameter casing, which will be set to approximately 136.
  • Petroset Cementing Service hooked up to the 8.6250 cement head and broke circulation. They then mixed and pumped Class A cement, with 3% calcium chloride. The cement, which was weighed during the complete cementing operation, weighed 15.5 pounds/gallon.
  • When the Contractor advised me they were circulating cement from the annulus.
  • Well shut in over night.
1930043295 2015-01-12 Orphan No Violations

This follow up inspection was conducted on 01-12-2015 for emergency contract compliance. 

 *The Ready Mix truck from Terminal Ready -mix Inc. arrived at 11:45 am with a load of 9 Sack Grout mix and was positioned over the well bore. The Contractor ran 1.5 diameter plastic pipe into the surface casing to approximately 149. This pipe will be used as a siphon string to remove any water in the hole while placing the grout. The grout was thinned with water to the proper consistency and was placed into the well bore. The extremely low temperature caused the siphon string to freeze up several times but we were able to thaw the line and set the grout properly. Approximately 2.5 cubic yards of grout was used to fill the well casing to surface.
2084518643 2014-12-12 Orphan No Violations

This follow up inspection was conducted on 12-12-14 for contract compliance.

  • (NOTE: On the 11th when the 6.75 diameter mill was tripped out and there was a
   visible ring on the flat part of the mill, this ring measured 2.375).


  • The drill samples showed cement, wood slivers, 2 piece of twine and white chips (soft).
  • During the last operation the contractor finished the fabrication of the ripping tool
    (Called an internal ripper).


Contractor shut well in and shut down till Monday.
