| 0176569273 | 2012-06-11 | Orphan | No Violations | At 9:00 AM, Mr. Polly (excavator) called and stated that he had excavated the well area and found that the vault with a 3 pipe had been filled with concrete. I told him that I will be there early this afternoon to advise him as to what needs to be done.
At 1:30 PM I arrived on location with DNR Inspector Ray Wallace. I advised Mr Polly to break the concrete up in the vaulted area to expose the well head. This 3 PVC pipe was installed inside of a 3-Bolt well head. Gas leaking from inside the 3 pipe was metered with a Lower Explosive Limit (LEL) of 11%. I advised the contractor to put the 4 PVC pipe over the 3 piping to vent until another vault could be supplied through the Orphan Well Program. I gave the Right-of-Entry form to Dan Polly, for Mrs. Plac to fill out. I recomend that Orphan Well Program only vault and vent this well. Gas reading - 11% LEL I&O case # 95-005 |
| 1339520408 | 2012-06-07 | Orphan | No Violations | I inspected this site due to a call from the City of Lorain Fire Department (CLFD) on the 6/6/12 of a gas odor in the yard of a rental property located at 1401 West 19th Street. I advised the CLFD to tell the renters to vent the 3 pipe, that was found during the excavation the renter was doing on the 6th, with a section of 4 PVC pipe..
During my inspection, I spoke with the renter (Mrs. Regler), who is renting this Section 8 apartment. Mrs Regler gave me the renting agents name and phone number (Mrs. Plac - 440-935-1598). I spoke with Mrs. Plac and explained that the area around the 3 pipe needs to be excavated to find the casing of the well. She said she will call the maintinance person (Dan Polly) and he will call before he starts doing the work. |
