1543417321 2018-08-10 Field Inspected, Well Not Found No Violations I conducted this compliance inspection at the request of Ms. Mary Kay Gates of the Village of LaGrange, due to a letter they received from the Division Surety/Bonding Section requesting filing fees that are due for this well (API #34-093-6-0389-0000). I met Ms. Gates on site to assist in locating this well. I conducted a file review of the ODNR database and the file contained a plat showing the location and a permit to deepen application issued on 01-2-1987 to the City of Lagrange which was cancelled by the Division on 02-12-1987 do to the fact the village had plans to rework this well to supply gas to the city office buildings located on the property. I had scheduled to meet with Luke Smith Survey Technician, from the Division Survey Section, to use a casing locator to locate the well and to obtain GPS coordinates. A metal reading was obtained at GPS: 41.230262, -82.120125. I advised Ms. Gates to hire a contractor to expose the well and for her to contact me when they have a date to excavate so the well can be evaluated.
