 2. Started to clean out at 13:00 hrs, working on the obstruction at 60 using a 4.3750 tri cone bit. At first the circulating fluid was clear but then it turned to a blackish color. Drilled down to 76.

 3. Circulating out large pieces of Red Bedford Shale, due to fresh water that is being used to circulate.

 4. Shut down for the day at 16:00 hrs.

0638603913 2014-02-11 Plug / Plug Back No Violations Visited location on 02/11/2014 with Norburt Lowder to check on status of excavation. Whipstock Drilling is continuining to excavate down to wellhead. At the time of the visit, they were at approximately 12 down from ground level.
0849949934 2014-02-12 Orphan No Violations A follow-up inspection was conducted on 02/12/2014 for contract compliance. 
  1. A replacement pump arrived and was placed close to the vault so  
      the water can be pumped from it. Once the waterwas pumped out of the 
      well vault and the casing was exposed, the gas leakage from the well 
      diminished.  No gas was detected in excavation, however elevated gas 
      readings were detected in the vault.  

  2.	Whipstock completed excavation down to the sewer line (approximately 



  3.	The sewer contractors arrived to determine if the lines needed to be 
     removed or worked around it. They decided to remove it.

1157642099 2006-09-07 Orphan No Violations Case # I&O 06-004, Met with Chris Teater to locate leaking gas well on housing development property at 37000 Center Ridge Rd. in North Ridgeville. Located a concrete column 23 underground with gas bubbling thru center of concrete column at 100% lel.. There is casing inside concrete but not sure of diameter. From photos it is 5 casing and it is bent over in bottom of 60 corregated plastic pipe over well . Construction company located gas leaking during removal of trees. Concrete around well will have to be chipped away to determine casing size. No houses are around well location at this time but construction to begin in October 2006. Sun, 70 degrees.
1170796367 2008-08-06 Orphan No Violations I&O case # 06-004 Chris Teeter. Requested by Mike McCormac to inspect well site for orphan well program. Well is in a new development and houses are getting closer to well site since last materix evaluation. Well has increased in gas leakage to 100 % lel 5 above well. My recommendation would be to plug the well this fall, to eliminate costs, due to hazards near a house. Nearest home now is 500 in diameter. I am submitting a new matrix to Mike McCormac. Dry, sun, 85 degrees.

1306816129 2014-03-26 Final Restoration No Violations A follow-up inspection was conducted on 03/26/2014 for contract compliance. 
   1)  This well is located in the next phase of a housing development of K. Hovnanian Homes. This well is on the south side of Tailfeather Drive, in the block of address 1088 to 1091. There is no paved street in this area yet. The housing contractor has installed the water and sewer main lines and several lateral lines.

1391692683 2014-02-03 Orphan No Violations Inspection on 02/03/2014 was conducted for contract compliance. 
 1.  The snow was removed from the area to be excavated.

     Corso, Whipstock Superintendent, to put orange flagging on each fence post and KEEP OUT 

 3. Excavating was started around well vault (black culvert). The projected excavation will be 

 4. All excavated material will be stockpiled at the perimeter of the site. The first stockpile will be 
     located in the southwest corner.

   Note: 

    a.	The Sewer line, which had been installed by the developer of the complex, is located 
       approximately 10-15 to the south of the well vault and at a depth of 18.

A follow-up inspection; 02/10/2014 was conducted for contract compliance.
    
   1.Whipstock is still excavating and stockpiling this material at the northwest corner of the 
      location.  This material will be used to backfill the excavation.

    2.Water is entering the excavation from the gravel fill at the base of the sewer line. A
       dewatering ditch was excavated along southern edge of the excavation to direct the water
       to the sump which is located in the southwest corner of the excavation.

1394469632 2014-03-10 Orphan No Violations Plugging and abandonment operations are being completed. Cementing was completed on Friday 3/7/14. The cement is firm and set at a nominal 24 inches below the top of the casing collar. Whipstock crews indicated that the plan is to cut the casing off at 4 feet below the existing grade and install a short section of black, corrugated drain pipe around the well once the cap is welded on with ID#. I inspected both the cement in the casing and samples of the cement retained in cups. Both look firm and solid. No gas detected via meter. I collected a GPS location data as well as triangulation measurements from selected locations for future reference.

   A follow-up inspection was conducted on 02/04/2014 for contract compliance.
   



  2. The water line on the north side of the excavation, that was installed by the developer was 
      exposed.Whipstock contacted the developer to shut water off. 

      east and west sides of the excavation.

Note: 

      When Whipstock was excavating in the area of the sewer line, which was previously installed by 
    the developer, they encountered black material that appeared to be high in organic matter (corn 
    stalks, vegtation, tree limbs and roots). Whipstock segregated this material and stockpiled it in
    the southwest corner of the location. This material will not be used as back fill.

1541231752 2014-03-20 Plug / Plug Back No Violations Visited well on 03/20/2014 to determine the status of the location at the request of Norburt Lowder. All rig-related equipment has been removed, and the location has been rough-graded. A 12 plastic culvert pipe is around the well, sticking out of the ground ~2. The water level within the pipe is down ~4 from the top of the pipe. Occasional bubbles were noted in the water in the pipe.
1608252901 2014-03-05 Plug / Plug Back No Violations Visited location on 03/05/2014 to observe plugging operations with Norburt Lowder. Whipstock Drilling had run in the hole with tubing to circulate out old oil in the hole. Will tag bottom with tubing and then prepare to plug out the well.

  A follow-up inspection was conducted on 02/19/2014 for contract compliance.

   1.	Plugging equipment removed from site.
   2.	Excavation was backfilled and compacted in 8-10 lifts. 
   3.	Site is ready for the Sewer Contractor to reinstall the sewer lines.

1894238965 2014-02-11 Orphan No Violations A follow-up inspection was conducted on 02/11/2014 for contract compliance. 
 1.	Pump problems due to sub-zero weather.
 2.	Whipstock continues to excavate and this material is being stockpiled at the northwest perimeter.

  Note: 	Cody Bruce, of FJD/Hovnanian Homes, arrived on site. He inquired about how the 
excavation will be backfilled.  I advised him to contact Gene Chini, Orphan Well Manager,
with any questions.
1981808815 2015-12-31 Final Restoration No Violations Collecting photographic evidence of final restoration. Final restoration is approved.
2011015490 2014-03-04 Orphan No Violations A follow-up inspection on 03/04/2014 was conducted for contract compliance. 
   1.Due subzero weather the equipment lines are freezing up.

   2. Whipstock order 2 load saltwater from Cargill to add to circulating half tank to keep water from 
       freezing. The salt water will help keep the Red Bedford from caving in on the  drill stem.

   3. Started to drill out with same bit 4.8750 on 2.3750 tubing at approximately 96.6 

   4. During drill out, at a depth of approximately 100, the well started to unload. The contractor 
     slowed their pump down to keep well under control, letting the well flow into half tank. The 
     initial flow back produced approximately 100 gallons of a thick black oil and then changed to   
     black water with a strong petroleum odor. I notified Asst Fire Chief Bement (cell 440-653-6362)
     of the odor and that he might get complaints from the area.
   5. I advised Whipstock crew to put a trap over half tank to keep the odor in.
 
   6.The crew ran tubing to 225 and shut down for the day.

2139366242 2014-03-07 Orphan No Violations I conducted this follow-up inspection on 03/07/2014 for contract compliance. 
  1.Confirmed that the half tank, to be used to contain flow back   from the plugging, was delivered.
  2. Tubing was run for first plug (1334 to 850) will cement with 110 sacks of Class A cement.
  3. Gel will be from 850 to 400.
  4. Cement plug from 400 to surface with 160 sacks of Class A cement.
