0356472551 2018-04-02 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1607. RBDMS indicated reported production was current. Access to the well was stable. No defined road to the well existed. Correct identification was present at the well head. The 8.625 well head was plumbed above grade and closed to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. The tank battery was shared with 1606, 1605, and 1608. A stable earthen dike that was free of fluid was present around the equipment. Equipment within the dike consisted of two (2) horizontal separators, two (2) 100 bbl production tanks and one (1) 50 bbl brine tank. Wells 1605, 1606 and 1607 produced into one separator. The separator remained in good repair and contained a current 16 day production chart. The production tanks remained in good repair. Correct identification was present. Valves were closed and plugged. Hatch lids were closed. The brine tank remained in good repair. Valves were closed and plugged. No violations were observed during this inspection.
1197899203 2007-12-14 Production Wells Violations Noted I performed a random field inspection. GPSd the well and the tank battery for the first time. I found oil and brine bubbling up from around the wellhead. I called Mitch Fitch and left him a message(330-495-3962). I then called his pumper Rodger(330-903-0695). Rodger told me that the well has had problems like this in the past. He would get a backhoe next week and try and find the problem. He called back in 2 hours to let me know that he had dug up around the wellhead and found no signs of a leak. But still needed the backhoe to dig deeper. I stated that it just needs to be repaired.
