At the time of my inspection Moore Well Services was on location to remove the plunger assembly and begin to convert the well to operate with a pump jack unit. I will schedule a follow-up inspection to document the status of the well and installation of screened security fencing for compliance with 1501:9-9 (E). No further action is necessary. |

I met with Duck Creek Energy representative Larry Gibler and discussed the scope of work and restoration of the location following the conversion. A workover rig operated by Moore Well Service was on location to remove the plunger lift assembly and replace the assembly with a pump jack unit. At the time of inspection the well had been blown down in preparation for the work and was no longer leaking. I will schedule a follow-up inspection to witness the repairs. No further action is necessary at this time. |
0732796657 | 2018-06-26 | Production Wells | Violations Noted | The purpose of my inspection was to conduct a status check inspection on well #2-2995. Inspector John Fleming was present during the inspection. RBDMS indicated reported production was current. Access to the well and tank battery remained drivable and stable through the Tod Cemetery. The well was located within a locked chain-link enclosure that contained correct identification. The 8.625 well head was above grade and closed to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. I witnessed gas leaking from the tubing head assembly. Cloth rags were tied around the assembly. No free fluid was observed on the assembly or ground. A stable earthen dike that was free of fluid was present around the production equipment. A locked and screened chain-link fence enclosure was present on top of the dike. Correct identification was present on the fence. Equipment on location consisted of one (1) horizontal separator and two (2) steel production tanks. The separator remained in good repair. No production chart was present. The production tanks remained in good repair. Valves were closed and plugged. A charcoal filter was present. Hatch lids were closed. While on location I called and spoke with Duck Creek Energy representative Larry Gibler regarding the well leak. Mr. Gibler stated he was aware of the leak and had scheduled a service rig to replace the tubing. I communicated to Mr. Gibler that I was issuing a compliance notice due to the well being insufficiently equipped to prevent the escape of gas. I requested that the surface leak be repaired within 14 days and to be notified of when the service rig was to be on location due to the urban location of the well. I will schedule a follow-up inspection to document the status of repairs. |

During the call I requested notification of when the rig arrived due to the urban location. I will schedule a follow-up inspection to document the status of repairs once completed. |
