0904088448 2018-04-25 Production Wells No Violations The purpose of my inspection was to conduct a status check inspection on well #2-3122. Inspector John Fleming was present during the inspection. RBDMS indicated reported production was current. Access to the well and tank battery remained stable and drivable. The well head was enclosed by a board on board fence that contained correct identification. The 8.625 well head was at grade. The production annulus was plumbed into the sales line. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. A domestic supply connection was present. The tank battery was shared with #2839. A stable earthen dike that was free of fluid was present around the production equipment. A dike drain with riser pipe was installed in the Southeast corner. A locked board on board fence that contained correct identification for both wells was present on the fence. Equipment within the dike consisted of one (1) horizontal separator and one (1) 140 bbl production tank. The separator remained in good repair and contained a current production chart. The production tank remained in good repair. Valves were closed and plugged. No violations were observed during this inspection.

1060636767 2010-11-06 Preliminary Restoration No Violations At 2:30pm I received a call from the Canfield Fire Department about a gas smell at this drilling site. I called the Fire Chief, Troy Kollar(330-261-1965). He stated someone was walking by the drilling location and smelled gas. I called George Strawn(330-727-1614) who was on the location with the Fire Chief. He stated the the starch from drilling started to rot and made the smell. There was no leaks around the wellhead. I arrived on the location at 3:30pm. I could not smell anything. A small part of the liner was down. About 4 feet was down on the southeast side of the drilling pit. I told George to pump the pit down below where the liner had dropped. I found no gas smell from the wellhead. But I did get some foul smell from the drilling pits. George pumped the fluid from the drilling pit to the frac tank. This complaint is resolved. The drilling pits will be closed on 11/7/10.
