Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1087926669 2004-06-03 Production Wells No Violations Inspected at the request of Harry Bircher of Western Reserve Land Consultants, Inc. No leakage. No violations. Well P/A 2-26-38.
1210003276 2008-05-05 Plug / Plug Back No Violations I received a fax from Rubino Construction, INC on 5/5/08. On June 22, 2004 Jonnie Patrick was called out to investigate this well. The contractors had hit the casing. Jonnie inspected the casing. There were no leaks around the cap. He instructed the company to fill in with dirt and he flagged the well. The contractors moved a huge rock over the well. Over time someone moved the rock and I was called out to find the well again. I GPSd the same as Jonnie Patrick did. I had an error of 13 on my readings. I met with the developer, Mr. Dominic J. Marchionda. I showed him where the well should be within the 13 radius. I flagged the same. He marked with a small rock. I stated to Mr. Marchionda that there could be an error within the 13 radius. He agreed. There were no violations at this time.
