0043906668 2014-04-24 Production Wells Violations Noted Inspected production well on 04/24/2014. The tank battery consists of 2-100 bbl steel tanks and a vertical separator located within an earthen dike. The tank battery is used by multiple wells; identification is posted for all wells utilizing it. The well is equipped with a pump jack with an electric motor located within an earthen dike. No well identification is posted at the wellhead. Research into the production history of the well showed that no production has been turned in to ODNR since 2012. Randell Oil was notified of the identification issue and will address it as soon as possible.
0519152554 2018-05-23 Production Wells No Violations I met onsite with Randel Oil Company representative, Randy Duncan, to set date and time for the clean-up operation. Originally planned to start on Thursday, 5/31. However, the Contractor is delayed until the 6/4. The site will be contained and monitored until then.
1529941961 2018-05-22 Production Wells Violations Noted I conducted this compliance inspection due to a phone call from John Smith, a Columbia Gas Field Operation Leader. He stated that during a service check of their line along Snowberry Lane they detected a possible line leak and dug an exploratory excavation to determine the source. I found they had excavated a 2 foot by 3-foot hole, approximatley 3-feet deep to find the suspected leak. The excavation start to fill with fluid that was coming from a layer of gravel under the base of the asphalt covering Snowberry Lane. This fluid consisted of water and what appeared to be crude oil. Columbia Gas ceased excavating and oil absorbent pads placed in the excavation. I called Randy Duncan to inform him of the incident and he stated that his pumper is on his way to evaluate the situation. I set up a meeting with Mr. Duncan for tomorrow morning.
1530296100 2018-06-04 Production Wells No Violations I conducted a follow-up inspection to oversee the clean-up contractor on site. The contractor started to excavate in same area as the prior excavation completed by Columbia Gas. They excavated to a depth of approximatley 5 feet where they encountered water and crude oil entering the excavation from a gravel zone. A water truck that was on site removed this fluid from the excavation. At a depth of approximately 4.5 feet, a 4-inch diameter steel pipe was exposed. The contractor thought that this may be a sleeve for the flow line. The contractor excavated and exposed both ends of the flow line and pumped fresh water from well to the tank battery. Prior to flushing the flow line, the contractor cut a hole in the side of the 4-inch diameter pipe to see if there was any communication between 2-7/8-inch flow line and 4-inch pipe during the flushing process. They pumped approximately 25 barrels of fresh water through the flow line, with no communication with the sleeve. The fluid in the excavation cleared up and no more crude oil was seen seeping out of the gravel layer. Fred Romeo from ODNR Emergency Response section was on site to oversee the backfilling and grading the excavation to ground level. The excavated area will be seeded at end of the month.
1531835151 2018-07-17 Production Wells No Violations Conducted a follow-up inspection on reclamation of site after oil seep was removed. The area disturbed in front of address 1505 has been graded and vegetation reestablished and the area to right of driveway of address 1500 that was disturbed during the line repair back in January of 2018 was regraded and top soil replaced and reseeded.

Case closed.

1576763167 2018-05-31 Production Wells No Violations I conducted an inspection to check on the contractors progress on the clean-up. OUPS has marked the utilities, the excavation that was done by Columbia Gas is fenced and additional oil absorbents pads have been placed on the fluid to absorb any assocoated crude oil. The site will be contained untill the contractor can start on the 4th.
