0179224870 | 2010-05-03 | Drill / Deepen / Reopen | No Violations | Witnessed mine string/surface casing. The following procedure was implented to resolve communication between the well bore and a room and pillar. It has been agreed upon, that the drilling of said well located in a coal bearing area, may have fractured a pillar resulting in large volume of air (possibly methane) being blown back to the surface. As a result, 371.10 of 7 casing was installed along with a cement basket at 289.5 and a Rag Packer (carpet) at 291. Total depth of 8.875 diameter hole was 422. The coal seam for this location was 330-337. Air flow from well bore had significantly decreased prior to cementing. Grouted backside of 7 with 80sks of 20/80 Poz. The cement eventually dropped approx 6. Waited 2 hrs before cementing bottom (inside 7). Checked fluid level with wireline and noted 288. Minimal air flow from inside 7 casing prior to bottom cement. 45 sks of 20/80 was then placed through 7 casing. The fluid level was again checked after completion of bottom cement and found at 288. Both procedures were accomplished with slow rate/low pressure pumping (via gravity). Note: company was unable to install observed plastic line with 7 casing to prevent air lock. Co. Rep Jack Greene states three attempts failed (not witnessed), due to kinking/coiling of line inside well bore. The backside of 7 was topped off with additional cement slurry to complete job. The cement stayed at the surface prior to leaving the location. |
0766195522 | 2017-11-13 | Production Wells | No Violations | Well is properly identified. The unit is straightened up and the stuffing box is no longer allowing gas to escape from the well. Violation has been resolved. |
0829754873 | 2010-05-07 | Drill / Deepen / Reopen | No Violations | 4.5 bond log. Timco Wire line logged TOC on backside of 4.5 @ 267. TD TOC inside 4.5 with tubing @ 1110 Mine 330 to 337. Cement top above top of mine. Service rig setting up to drill out cement in 4.5 to TD. |
1034090248 | 2011-02-23 | Preliminary Restoration | No Violations | Good identification on tank ladder. Pit has been filled in. Site graded. Vegetative cover established. Well is equipped with tubing, rods, pump jack, gas engine, connected to mechanical separator, 100bbl steel tank, meter, and production line. |
1101262016 | 2017-11-01 | Production Wells | Violations Noted | This well was not properly identified with the current companies name and phone number. This well will need properly identified. The stuffing box is leaking gas around the polish rod. The pumping unit has slid forward and is pushing on the polish rod causing it to bow. This excessive pressure on the stuffing box appears to be the cause of the leak. I called Jody Jasper at 12:15 pm and left a voicemail explaining the notice and the issues for this well. |
1275392733 | 2010-04-29 | Drill / Deepen / Reopen | No Violations | Arrived at well location to observe approx 80 bbl saltwater being pumped to eliminate a constant volume of air being blown back to the surface. This procedure was not successful. The air exists in locations inside and outside the drill pipe. The rig crew states that earlier this morning air blew much harder, as mist of water was observed halfway across the pit. Currently, the well bore condition is approx half of the said claim with a steady stream of water (filled 250 ml bottle in approx 6 seconds). A gas detector showed no signs of methane at this time. The geolograph showed 464 with no installed surface casing. The second procedure began at approx 11:45 am and consisted of pumping gel, clay, super mud, cottonseed hulls and flake into the well bore. During this procedure, the air pressure would decrease slightly but only to return to existing condition. The rig crew would then remove rubber and air hammer. At the catwalk location, the gas detector displayed an LEL 3-4 for a couple of seconds before returning to zero. Contacted the Jackson Office to inquire about well location and related history of coal activity. Spoke with Mr. Greg Mills, Supervisor, at the Jackson Office. Mr. Mills provided contact information for Mr. Larry Bean at 740-669-7721 (ext 7719) and Michael Lively with Southern Ohio Coal. Notified Mr. Bean of well bore condition. Mr. Bean states the said location was room and pillar mined, so blocks of coal were left; believes company may have hit those when going down through void; bottom of coal elevation 422, surface down the coal seams would be 330 (in the mine) to 337 (bottom; out of mine). Mr. Bean does not believe there would be water in voids because, injection holes across the road are still dry. Further stating that it takes low barometric for blow back. The contacts for Southern Ohio Coal were provided to Mr. Greene. Relayed this information to Mr. Greene. He would like to see if the pressure drops after a day. Instructe |
