1339683109 2019-02-13 Production Wells No Violations On this day, I am accompanied by ODNR Inspector William Stolarik. I received a call from landowner, Glen Stephens on 2/12/2019, stating that a slip had occurred on his property that incorporated a gas line associated with this well. He informed me that there was no incident at the time, but wanted to prevent an incident from occurring. During the inspection, no slip was found around the well. The well resides on flat, low-lying land near Skin Creek. I found the well operated as an open-flow well. The commercial sales line has been disconnected, but a flowline providing domestic gas is currently attached. The pressure gauge attached to the wellhead has a reading of approximately 120 pounds per square inch. Contact with Mr. Stephens will be made, and further investigation of the slip will be addressed.
1459429064 2016-03-31 Production Wells Violations Noted Production well status check inspection found the well equipped with 5.5 production casing swaged down to a 2 sales line. Also noted was the 2 commercial sales line appears to have been severed by mechanical means and the well head altered in such a way as to supply domestic gas to the landowner only, with a domestic gas hook up present and 120 psi showing on the gauge. There is no production tank present. The well identification reflects the previous owners information. Also noted is historically the landowner contracted a Ron Early who contacted me on 02/26/2015 in regards to the landowner being out of house gas, Mr. Early indicated His intentions to alter the well head, I informed Mr. Early that the well was private property and He could only make changes to the well head with directions and permission from the producer. The producer Jeff Miley was contacted on 02/26/2015 and informed that Ron Early was intending to alter the well head and the landowner was out of house gas. Instructions are as follows -post proper well identification within 30 days as discussed with the producer Jeff Miley in a phone conversation on 03/31/2016 at 13:15 hours.
1550078525 2019-02-13 Production Wells No Violations On this day I accompanied Inspector Tom Thomas on site to follow up on a recent complaint received by the landowner. The complaint expressed concerns regarding a earthen slip caused by a gas line. No earthen slip was observed in the area of the well. The well is situated behind the residence on the property near a stream. A domestic hook up and pressure reducing valve is present. The 2 diameter commercial sales line was previously severed by a contractor hired by the landowner and has not been repaired. Proper well identification is posted.
1709726711 2019-03-01 Production Wells No Violations On this day, I am conducting a follow-up inspection per request of landowner Glen Stephens, in regard to a slip that has occurred on his property, and incorporates a gas line of the Miley Gas Company (owner 5429). The line is not directly associated with this particular well, but does traverse Mr. Stephens property from a conjoining property, to reach the sales meter nearby. The slip occurred approximately 1,000 feet North of Skin Creek Road, on Mr. Stephens property. The gas line is caught around a tree that has moved downslope approximately 30 feet from its original position, tightening the line. The line has not yet been compromised, and is still in tact. An attempt to contact Miley Gas (740) 509-1403, was made on 3/1/2019, with no success, and unable to leave a voicemail. Multiple photographs were taken. As for this well (2-0345), Mr. Stephens stated that it has been in litigation, and he is attempting to claim ownership.
2034238477 2015-02-26 Production Wells No Violations While driving by the well I noticed a roustabout crew working on the well. I stopped to investigate the scope of their work. The crew was replacing a house gas line for the landowner. My inspection of the well site finds the well is equipped with 5 casing and produced by open flow method. The identification needs to be updated. The owner of the well was notified of the identification error.
