0775193530 2017-10-05 Plug / Plug Back No Violations I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. Harmons Well Service was on location and Formation Cementing was on location to finalize the surface plug. The cement was tagged at 30 feet and the well was topped off with 18 sacks of cement.
1425400636 2015-03-03 Production Wells Violations Noted Status check inspection as a response to complaint # 4951 filed by Landowner Jane Percel. Upon inspection the following equipment was found at the well head-6.625 casing-2tubing and pumping rods with no pumping unit attached to the well head and no production flowlines or valve present on the tubing or casing. The State records show reported production for 2013 by Harmons well service. The current condition of the well finds it incapable of commercial or domestic production with no well identification posted at the well head. Instructions are as follows-Plug the said well within 180 days. ODNR Monroe County Inspector Bill Stolarik 740-507-6592.
1427243842 2015-03-24 Production Wells No Violations Report to document the phone conversation between the landowners Jane Percel -Janice Reynolds and myself on this day with both parties contacting me in regards to the status of the well and complaint # 4951. Both parties have been informed I inspected the well on 03/02/2015 as part of their complaint and issued a violation identified as # 1425400636. It is my understanding CGO Inc has contracted Harmon Well Service to evaluate the said well and has a rig on location addressing the violation issued. The due process of the law has been explained to both parties on this day.
1427816038 2015-03-31 Production Wells No Violations Status check follow up inspection to complaint # 4951 and violation # 1425400636. Upon inspection of the well head it was observed that Harmons Well Service rig is on the hole and has pulled the 2 tubing and pumping rods out of the hole. A string of 4.5 casing has been moved in on location. Contact was made with Troy Harmon who indicated that they would be running the 4.5 and setting it on a packer. ODNR Regional Supervisor Brent Bear was called and updated on the current well activity.
1429114068 2015-04-15 Production Wells No Violations Status check follow up inspection to complaint # 4951 and violation # 1425400636. Harmons Well Service has ran 1459 of 4.5 casing in the hole and set it on a Anchor Manufactured compression set down packer. The well bore fluid level was at 700 and total measured well bore depth was at 1,694. The service rig is currently running 2 tubing in the hole. No production flowlines were noted as present on location related to the said well.
1429654654 2015-04-21 Production Wells No Violations Status check follow up inspection to complaint # 4951 and violation # 1425400636. On this day it was observed that Harmons Well Service has now installed 4.5 casing set in the well bore on a packer. 2 production tubing and pumping rods are present along with a pumping unit. The well repairs are now complete and new flowlines are being installed to an existing tank battery.
1431967113 2015-05-18 Production Wells No Violations Status check follow up inspection to complaint # 4951 and violation # 1425400636. Upon inspection it was observed that the well has had repairs made in the extent of the installation of production tubing set on a packer with pumping rods installed and a pumping unit set on the well with production valves in place. The owner has Harmons well service contracted to pump the said well.
1435255532 2015-06-25 Administrative Inspection No Violations Status check follow up inspection to complaint # 4951 and violation # 1425400636 administrative report to document the initial violation was resolved on 05/18/2015 by the means of the producer making the proper repairs to the said well and putting it back into production.
1438276518 2017-10-03 Plug / Plug Back No Violations 10/03/2017 - Inspector Bill Stolarik on site to witness the setting of a 300 plug to isolate the well bore inside the 7 diameter surface casing above the shot hole. The company reported that the tagged the plug set on 09/29/2017 at a depth of 1,188 and had 2 tubing setting in the well bore at a measured depth of 1,080. A 300 plug was set consisting of 57 sacks ao class A cement with 1% gel and 1% calcium additives, the total slurry plug volume was 12.7 barrels. The tubing was pulled up to the next plug to be set at 300 and the plug allowed to cure and was tagged at 980. A 30 barrel surface plug was set through 2 tubing from 300 to 20 with the well circulating and holding fluid but not circulating cement before Formation Cementing ran out of bulk product. The well circulated behind the 7 casing at surface indicating were were also filling up behind the 7 with cement. The 2 diameter plugging string was pulled and had cement on the annular of the tubing. The plug will be tagged the next working day.
1467820377 2016-07-06 Production Wells No Violations Status check inspection found the well equipped with 6.625 casing-4.5 casing -2 production tubing and pumping rods with the well set up to be produced by the means of a pumping unit, the casing valves are in the closed position and the company has not produced the well since being served with a injunction from Eclipse. At the production fluid storage area 2-100 barrel production tanks are present in a established dike area. On this day I spoke with the producer Ed Crum in regards to the 2 wells on the lease. Mr. Crum indicated He would like to plug both wells and would get His attorney to address the injunction through the court system. Mr. Crum also indicated He would file for plugging permits for both wells. I also visited with the landowner and informed Him I would be inspecting the wells on the property and the injunction did not apply to the Division.
1507122213 2017-09-29 Plug / Plug Back No Violations
1507217015 2017-10-05 Plug / Plug Back No Violations Status check inspection on the plugging process to confirm the top of cement on the surface plug found the cement level from the previous surface plug at 30 measured depth from surface. The well was topped off by Formation Cementing with a 4 barrel cement mix consisting of 18 sacks of 15.2 pound per gallon and 1.26 yield cement. Also previously noted was the existing 5.187 casing in the hole was the only string present and was separated at approximately 300 down hole from surface. We did circulate cement both internally and externally behind the 5.187 casing in which was the only string present at surface. The well plugging is complete on this day.

Harmons Well Service was on location and Formation Cementing was pumping a 300 foot plug consisting of 57 sacks of Class A cement, with 1 percent gel, and 1 percent calcium additives.

Harmons Well Service was on location with a service rig and Formation Cementing had previously pumped 100 sacks of cement to bottom. Tagged cement at 1,188 feet.

1975528969 2018-06-19 Final Restoration No Violations Status check inspection found the well site restoration meets ORC 1509.072 with vegetation established and erosion control in place. All equipment has been removed.
2029967353 2017-09-25 Plug / Plug Back No Violations Status check inspection found Harmons Well Service rig on the hole preparing to begin the well plugging process. They have pulled the pumping unit off the well and also pulled the pumping rods from the well bore and are preparing to remove the production tubing from the well. A plugging permit has been issued for the well.
