0515096588 2012-07-19 Production Wells No Violations On the day of my inspection I was accompanied by William Stolarik and Cynthia Vandyke. The pumping unit was actively pumping and we followed lines leading from the well head down the North East slope to the Witten Fork Creek. At the creek there was a 1.5 inch line dispersing production water into the creek. We obtained samples at the 1.5 in line and at various points down stream. The production water reach the maximum reading on the QuanTab at all sampling points. Samples were taken to the Cambridge lab for further evaluation.
0658890734 2012-10-25 Production Wells No Violations Follow up inspection pertaining to active compliance notice found the following work has been done. Vegetation has been established below the well site but still needs ample time to vegetate at the tank battery site, A single 100 barrel tank has been set but is not hooked up and the well is not in production. It is the understanding of the inspector that the well is need of down hole repair and a service rig is scheduled to be brought in to complete this work. The inspector asks the producer to please notify Him 24 Hours before commencing these operations. Inspector Bill Stolarik 740-507-6592. Inspector will note that the producer is complying with the terms stated in the initial violation issued and once the remedial work has been completed and the well is capable of production the production waste water is being properly disposed of the compliance notice will be resolved.

Inspector will extend compliance order in allowance for vegetation to establish and producer to have a resonable amount of time to build tank battery site.

1342741015 2012-07-10 Production Wells No Violations On the day of my inspection I was unable to perform a proper inspection due to the abundance of Asplundh equipment and workers. I will contact Monroe County Inspector Bill Stolarik and Cynthia Vandyke and set up a time to do a proper inspection and collect data if necessary.
1342781469 2012-07-19 Production Wells No Violations Inspection and investigation of well site in regards to an anonymous brine dumping complaint received on 07-09-2012 by Inspector Cindy Vandyke. Inspector William Stolarik met with inspectors Cindy Vandyke and Andrew Corder and traveled to well site to conduct an inspection of the said well. With the following actions take. Upon location of well head a GPS reading was taken identifying well head to coordinates in the RBDMS database and the following equipment was present and noted. 7 surface casing, 2.875 intermediate string, 1.5 production tubing, .580pumping rods and the well being produced by the means of a pumping unit, with the production valves open and the production lines separated in a way as so no fluid separator would be needed to separate production fluid and natural gas from the well bore. Also on site directly in front of well-head are 2 gas meters with active sales charts in plain site. Inspector will note pumping unit was operating entire time inspectors were on location approximately 4 hours. Upon investigation of area around well site a kill area with the approximate size of 100 by 400was discovered over the embankment. With the 1.5 Plastic fluid production flow line hooked up to and running from the tubing valve and over the hill onto of, and past the kill area which indicated to Inspectors this kill area was older and there are 2 sites contaminated on location. Inspectors then initiated a grid search to try and find the production fluid tank or the end of the flow line. With no production tank found related to said well. After an extensive search of the area and by directly following the flow line from the well head to the end of the 1.5 tubing flow line the following was discovered. Inspector Stolarik found the 1.5 tubing flow line being allowed to discharge production water directly from the well bore into a wet weather stream bed with noticeable discoloration and lack of vegetation in streambed. At that point Inspectors made the decision to
1342999022 2012-07-19 Production Wells No Violations ODNR received anonymous complaint that Carl was dumping brine in the creek. Upon arrival, I, Andrew Corder and Bill Stolarik found pumping unit running and no tank or separator. By following line down over the slope towards the creek, we found where 1.5 line came out into the creek. We followed the creek downstream about 1500 taking samples and testing the water with Quantabs. Using the high-scale Quantabs, all tests except last one were off the scale (last one tested 9 out of 10) indicating high amounts of chlorides. Four different water samples were taken - one right out of the discharge hose - all were put on ice to take to lab. There was no water in the creek above the discharge so did not get a control sample this day. We called OEPA and informed them of situation. Bill Stolarik is Monroe County inspector and will proceed with data collection and prosecution - see his inspection for more detail.
1343067222 2012-07-23 Production Wells No Violations Inspectors Andrew Corder and William Stolarik met on location with well owner Carl Heinrich and His field representative Brian Chavez in regards to production fluid surface contamination. Mr. Heinrich has been order to stop fluid production onto surface and clean up well site and affected areas and not to resume production of well until Violations have been resolved Per Southeast supervisor Rock King.
1343089586 2012-07-19 Production Wells Violations Noted An anonymous complaint was received by Division in regards to production fluid being discharged into an intermittent stream. Upon inspection of well site, the well was found to be in production and production fluid being discharged into an intermittent stream that flows directly into Witten Fork Creek with obvious signs of surface contamination. Inspectors Corder and Stolarik met with producer Carl Heinrich and Brian Chavez and ordered the said well to be shut in and remedial work to commence immediately to affected soils caused by oilfield contamination. The well was ordered by the Chief of Division of Oil & Gas not to be put back into production until the remedial actions have been approved by the Division. Work on site to be addressed as follows; cease surface discharge, well fluids are to be produced into production fluid tanks in a properly diked and identified tank battery not to be located near any waterway, clean up all debris, any affected soil that will not support vegetation is to be removed from site and hauled to an EPA approved landfill, and establish vegetation and proper long-[1509.22.6] of the Revised Code; in association with a method of enhanced recovery as provided in section 1509.21 of the Revised Code; or by other methods approved by the chief for testing or implementing a new technology or method of disposal. Brine from exempt Mississippian wells shall not be discharged directly into the waters of the state.

1366813262 2013-04-24 Production Wells No Violations Follow up inspection on previous resolved pollution and contamination found the well being produced in accordance with the terms of the previous compliance notice. 2 gas meters are present at the well head one reading Benwood house gas, Upon observation of multiple domestic services and conversation with multiple residents throughout this portion of Lee and Green townships I have been able to determine that Heinrich Ent is commercially selling and providing gas as a commercial provider to well over 20 residences in the area from this said well.
1574641296 2012-12-10 Production Wells No Violations Inspection of the well site found a service rig belonging to the well owner Carl Heinrich on the hole and attempting to change out the down hole pump. Inspector counted the pumping rods laying on location that were removed from the well and a total of 74 rods were present. At 25 per rod would put the well depth to be at least 1900. Compliance agreement conditions are being met by establishing vegetation and the setting of a production tank and the building of a tank battery.
