0148927268 2018-06-19 Final Restoration No Violations The said well was plugged on 03/26/2018. I was in the area but did not witness the plug job due to a time delay by the plugging contractor. A new lease road was cut off an adjoining property and has not been restored.
0596039820 2018-03-26 Plug / Plug Back No Violations On site inspection to witness the well plugging process. At 15:00 hours on 03/24/2018 I was contacted by a contract worker for the well owner identified as Chuck Dye who indicated they would like to plug the said well on Monday 03/26/2018 at 09:00 hours. Mr. Dye has been previously informed that a plugging plan must be submitted and approved prior to commencing any plug jobs. Mr. Dye informed me that the 6.625 was previously set to 1,460 and was cemented with 20 of cement to cover the Keener sand formation from 1,433 to 1,443. The well is a Berea Sand formation open shot hole completion drilled to 2012. Mr. Dye stated that his service rig had ran 2 tubing to bottom and he wished to commence plugging Monday morning. I referenced our previous conversations regarding not knowing the top of cement or isolation behind pipe and informed Mr. Dye the well should of been bond logged and perforated. Mr. Dye showed objection and after 1 hour on the phone I informed him they were responsible for any failed cement jobs and they would have to drill the cement out if the well leaked. Inspector Tom Thomas and myself arrived on location at 09:00 hours to witness the plug job to find the cement contractors identified as Formation Cementing along with Monroe Drillings water truck with Mr. Dye or His crew present. We waited 1.5 hours with no sign of Mr. Dye. At that point I contacted my Supervisor Mike Moreland and informed him of the entire situation including the plan to plug. I left location with Inspector Tom Thomas to conduct further inspections and returned at 13:00 to find the contractors still not ready. I once again spoke with my Supervisor Mike Moreland and advised him that I told the well owner Pat Shapley that they were responsible for any failed plug jobs.
0727572572 2015-11-17 Production Wells No Violations Status check inspection found the following present at the well head-5 casing -2 tubing along with a 1 vent packer string with both the casing and tubing flowlines hooked up and in place and the well being produced by the means of a pumping unit. A single 40 barrel production tank is located on location near the well head. Also noted is the well is being used primarily for domestic house gas purposes and the producer Mr. Pat Shapley who accompanied me on this day indicated the well was originally drilled in 1914 to the Kenner sand formation and deepened to the Berea Sand in 1954.

I attempted to contact Mr. James Shapley, and on 6/11/2019, I spoke with Mr. Shapley, and he informed me that he will be removing the equipment immediately.
