0204580040 2015-08-10 Production Wells No Violations Status check inspection on compliance notice # # 1426493928 found no attempt has been made to remediate the affected area inside or outside the tank battery or around the well head where pollution and contamination is present. I did speak with a service contractor by the name of Chuck Dye who indicated he has been contracted to bring in his service rig and pull the existing production tubing and evaluate the 6.625 casing to see if there is a hole that can be isolated by running a isolation string on a packer. Once the evaluation is done the decision will be made to plug the well or produce it. The well is currently producing gas only and is not being pumped or fluid being discharged.

On this day it was observed that the pollution and contamination observed in my previous inspections remains present and not addressed. No contact has been received from the producer. The issue was addressed with Aaron Wolfe with the Ohio EPA.
0557605198 2015-03-25 Production Wells No Violations Violation # 1426493928 follow up inspection with Regional Supervisor Brent Bear accompanying me on this day with the following observed- A scald are was observed both inside and outside the production take storage area with a brine scald and oil residue also on the earths surface below the well head. The production casing was uncovered to confirm the well did have 4.5 casing to allow me to time date the well for identification purposes.
1045645909 2015-04-22 Production Wells No Violations Compliance Notice # 1426493928 follow up inspection. On this day I visited the site with Aaron Wolfe with the Ohio EPA to observe the offsite impact caused by the surface discharge of the production waste water. It was noted that since my initial violation was issued on 03/15/2015 no effort has been made to remediate the affected area inside or outside the tank battery containment dike area or around the well head. I did on this day contact the producer by phone and explain that the violation had expired and question when they would remedy the problem, Ms. Christman indicated they were waiting on Chuck Dye.
1391248770 2016-03-29 Production Wells No Violations On site inspection follow up to compliance notice # 1426493928 issued against the said well found the well has had a 2.875 string set in the well bore to isolate a hole in the 6.250 in which addressed the issue with the well making large amounts of water. Addressing the pollution & contamination at the tank battery- There has been a 1,200 gallon tank set to recover any water that may be generated from the water separator during production and the area affected previously affected by the fluid discharge has been addressed. Photographs were taken on this day and the previous violation resolved.
1426493928 2015-03-15 Production Wells Violations Noted Status check inspection found the following equipment at the well head 6.625 casing - 2 tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit. At the production fluid storage area a single 100 barrel production tank is present with a fluid knock out separator inside a containment dike. On this day I visited the site with Region UIC Inspector Dave Ball and a scald area was observed and photographed inside and outside the dike area. It was also observed that the fluid knock out separator was being allowed to discharge inside the dike area with visible scalding both inside and outside the dike area. Instructions are as follows -Stop the fluid discharge onto the earths surface immediately-Remediate the affected area by the fluid discharge at the tank battery and wellhead. 1429051837 2015-04-14 Production Wells No Violations Compliance Notice # 1426493928 follow up inspection. Upon inspection it was observed that no attempt has been made to address the pollution and contamination on the inside or outside of the tank battery dike or around the well head. No attempt has been made to repair the production equipment to stop the fluid discharge onto the earths surface. Contact was made with Aaron Wolfe with the Ohio EPA and a day has been set the following week to look at some of the ongoing issues in the area.

1452613000 2016-01-12 Production Wells No Violations On site inspection follow up to compliance notice # 1426493928 issued against the said well found a service rig on location owned by Chuck Dye. A Gamma Ray log was ran by JET Wireline indicating a top of cement behind the 5.5 at 1,256, the wireline also confirmed the well bore total depth of 1,825 and holes in the 5.5 at 750. The rig has ran a string of 2.875 casing into the well bore to a depth of 1,631 with a packer set inside the 5.5 on the bottom of the 2.875. 1.5tubing was set to 1,814.05. The well bore fluid level prior to setting the packer and isolation string was at 925. The well bore perforations were confirmed in the Berea at 1,788 to 1,794. The rig will run pumping rods back in the well.
