1100180986 2018-09-05 Production Wells No Violations On this day, I am conducting an inspection per request of landowner, Ross Thomas. I am accompanied by Mr. Thomas during the inspection. He has indicated that the stream crossing, near the well, hasnt been maintained properly, which creates the stream to divert around the culvert, during a rain event. The stream bed is currently dry. The well is equipped with 15 inch diameter conductor casing with open annular space between it and the 5.5 inch diameter casing. There is fluid present within the annular space, approximately 4 feet below ground level, and occasional bubbles release through the fluid. There is 5.5 inch diameter production casing and 2 inch diameter tubing with the production valves open. It is produced by means of a pumping unit with rods and a gas engine. It will be noted that the 2 inch steel line connected to the casing, appears to lead toward a 10 foot section of approximately 7 inch diameter casing that terminates in the stream bed. The casing is sealed on the end with a 1 shutoff valve and bull plug. There is no indication of pollution and contamination during the inspection. The production facility consists of (1) 100 barrel steel tank, not contained within an earthen dike. The identification at the tank is faded and will need updated, and the production records are current. Pictures were taken during the inspection. I spoke with Rosalie Christman at Christman Supply on 09/05/2018 at 12:45pm, and explained my findings. She indicated that recent work had been conducted on the culvert to address stability issues that were created with the excessive rain this year. She was also informed to address the identification issue, and the construction of an earthen dike surrounding the tank.
