0354488994 2013-08-12 Production Wells Violations Noted Notification was received by Trevor Erwin of the Ohio EPA of a possible oil spill in the area. Contact was made with Triad and upon meeting with Bobby Handschumaker it was determined that there had been a release of 4 to 6 barrels of crude oil from the 100 barrel production storage tank due to equipment failure. The company had recovered most of the product upon inspection. Directions are as follows, Continue to monitor the site and recover all product, Erect proper containment around the tank battery according to SPCC rules, In the future make proper notification to the Division according to Ohio Revised Code when any product is released 25 gallons or greater outside the containment area or 200 gallons or greater inside the containment. 1376320197 2013-08-12 Production Wells No Violations 7:30 AM-Notification was received from Trevor Erwin from the Ohio EPA in regards to a possible oil spill called into the Federal EPA on 08/11/2013. I made several phone calls and was able to determine that there had been a spill in the area of the well on that day. I called Triad and met on location with Bobby Handschumaker at 9:00 am on this day. Bobby stated that the bottom of the 100 barrel tank had started to leak and released approximately 4 to 6 barrels of oil in the dike with part of the product getting outside of the dike. The company had most of the product recovered upon my inspection and will continue to monitor the site and recover the product. The company will also rebuild the containment dike according to SPCC rules.
1376405695 2013-08-13 Production Wells No Violations Follow up inspection to check on the status on the clean up of a oil spill from 08/11/13. Inspection found that the company has spill rags and boomers in place and is recovering the product as it migrates into the collection points. Bobby Handschumaker was on site along with 2 other Triad employees. I was also notified on 08/12/13 in the late afternoon that the company had found a point of impact on the Little Muskingum River with a slight sheen of oil on the water, the company has boomers in place on the river.
1698801343 2013-09-04 Production Wells No Violations Follow up to compliance notice # 354488994 found the oil spill has been cleaned up and tank battery rebuilt in a manner sufficient to hold fluid. Violation resolved.
